Screening for silent aspiration in hyperacute stroke: A feasibility study of clinical swallowing examination and cough reflex testing
CONCLUSIONS & IMPLICATIONS: CRT, CSE and FEES are feasible in 58% of hyperacute stroke patients in this setting. FEES anxiety is the main recruitment barrier and is not always well tolerated. Results support further work to establish optimum methods and differential sensitivity/specificity of CRT and CSE in hyperacute stroke for SA identification.WHAT THIS PAPER ADDS: What is already known on this subject SA significantly increases the risk of pneumonia in the early days post-stroke. CSEs are unreliable for identification of SA risk in this population. CRT is gaining popularity as a potential tool to identify stroke patien...

Patients with Stroke and Dysphagia: Demographics and Trends in Percutaneous Endoscopic Gastrotomy Tube Use in Stroke Patients in the United States from 1988-2010 (P01.219)
CONCLUSIONS: The use of PEG tubes has increased over time in US hospital admissions for acute stroke, and was associated with multiple comorbidities, greater length of stay and age > 74. This shows the increasing awareness of post-stroke dysphagia.Disclosure: Dr. Johnson has nothing to disclose. Dr. Dubinsky has received personal compensation for activities with Allergan, Inc. Mechanical Ventilation for Acute Stroke: A Multi-state Population-Based Study
Conclusions In this large population-based sample, over half of mechanically ventilated stroke patients died in the hospital despite the fact that younger patients were more likely to receive mechanical ventilation. Future studies are indicated to elucidate mechanical ventilation strategies to optimize long-term outcomes after severe stroke.

Stroke Impairs Bactericidal Function in Phagocytes (P5.131)
CONCLUSIONS. Oxidative burst and NETosis were reduced in cells derived from stroke patients compared to controls. In vitro, NETosis was inhibited by the addition of adrenalin and dexamethasone to the cell culture. Migration, phagocytosis of bacteria and the release of HNP 1-3 were not altered in stroke patients. We conclude that the immunological consequences of stroke are not limited to the adaptive immune system but extend to the bactericidal function of phagocytes. This may be causally related to the enhanced susceptibility to infection in stroke patients. The hormone induced inhibition of NETosis in vitro further suppo...

Establishment Of Stroke Ward Results In Immediate Reduction Of Major Complications (P5.143)
Conclusion: A protocol based multidisciplinary care Stroke Ward care significantly reduces common early complications of acute stroke. It also helps in significantly reducing the length of stay, saving total bed days at a tertiary care hospital, hence improving the overall care of these patients.Disclosure: Dr. Akhtar has nothing to disclose. Laryngeal Elevation Velocity and Aspiration in Acute Ischemic Stroke Patients
This study aimed to explore the ability of laryngeal elevation velocity to predict aspiration in patients with acute ischemic stroke. MethodsThis was a prospective cohort study that included consecutive acute ischemic stroke patients treated at a teaching hospital during a 10-month period. Patients underwent magnetic resonance imaging (MRI) to confirm the diagnosis of acute ischemic stroke. Patients who were at risk of aspiration and could swallow 5 ml of diluted barium (40%, w/v) for a videofluoroscopic swallowing (VFS) study were included. The association between abnormal indices in the oral and pharyngeal phase of the V...

Research on the Cause of Death for Severe Stroke Patients
ConclusionsThe most common cause of death in stroke patients was damage to the central nervous system. Independent risk factors were brain herniation, multiple organ failure, dyslipidemia, community‐acquired lung infection, the use of mechanical ventilation, hypoproteinemia, a history of hypertension, and hospital‐acquired pneumonia. Clinicians should be aware of the presence and possible effects of these conditions. Early prevention, monitoring and intervention to modify controllable risk factors will improve patient prognosis. Relevance to clinical practiceClinicians should be aware of the multiple independent risk f...

Predictors of dependence after MCA ischemic stroke submitted to thrombolysis.
DISCUSSION: .Dependence is an important indicator of the efficacy of stroke care. Besides it was not possible to evaluate rehabilitation, this is the first study on predictors of post-stroke dependence that focus exclusively on patients with first-ever MCA IS submitted to IVT. NIHSS, LAA stroke, in-hospital pneumonia and seizures after discharge were associated with dependence after first-ever MCA IS submitted to IVT. PMID: 29125072 [PubMed - as supplied by publisher]
