Clinical Features and Voxel-Based-Symptom-Lesion Mapping of Silent Aspiration in Acute Infratentorial Stroke
This study is a retrospective observational study based on a prospectively collected FEES and stroke database. Consecutive patient cases with acute ischemic infratentorial stroke and FEES examination between 2017 and 2022 were identified. Group allocation was performed based on PAS scores. Imaging analysis was performed by manual assignment and by  VBS/ML. Group comparisons were performed to assess silent aspiration characteristics. Binary logistic regression analysis was performed to determine if baseline clinical, demographic, and imaging parameters were helpful in predicting silent aspiration in patients. In this study...

Systemic Inflammation Exacerbates Stroke in Patients with Left Ventricular Assist Device (P4.235)
CONCLUSIONS:In LVAD patients, total prevention of cerebrovascular complications cannot be accomplished only by adjusting warfarin dosage to meet the therapeutic range. However, recognition of increased CRP and appropriate management of inflammation prior to the stroke onset may enable us to avoid its aggravation and secure patient’s opportunity for heart transplantation.Disclosure: Dr. Ohtomo has nothing to disclose. Outcomes after surgery in patients with previous stroke
ConclusionPatients with previous stroke had a higher risk of adverse postoperative outcomes; their 30‐day in‐hospital mortality rate was nearly twice that of patients without previous stroke.

Parenteral fluid regimens for improving functional outcome in people with acute stroke.
CONCLUSIONS: We found no evidence that colloids were associated with lower odds of death or dependence in the medium term after stroke compared with crystalloids, though colloids were associated with greater odds of pulmonary oedema. We found no evidence to guide the best volume, duration, or mode of parenteral fluid delivery for people with acute stroke. PMID: 26329401 [PubMed - as supplied by publisher]

Codeine is associated with poor prognosis in acute stroke
ConclusionUse of codeine‐containing analgesics is associated with a poorer short‐term prognosis and an increased occurrence of complications in the acute phase after a stroke. The highly significant findings suggest that codeine has a negative effect on acute stroke patients. The study reflects exploratory analyses and prospective studies are necessary to determine the background of the association observed in our study. Use of codeine‐containing analgesics is associated with a poorer short‐term prognosis and an increased occurrence of complications in the acute phase after a stroke. The highly significant finding...

Minimal Setting Stroke Unit in a Sub-Saharan African Public Hospital
Introduction: Sub-Saharan Africa (SSA) has the highest stroke prevalence along with a case fatality that amounts to 40%. We aimed to assess the effect of a minimal setting stroke unit in SSA Public hospital on stroke mortality and main medical complications. Materials and Methods: The study was set in Conakry, Guinea, Ignace Deen public referral hospital. Clinical characteristics, hospital mortality and main medical stroke complications rates (pneumonia, urinary tract infections, sores and venous thromboembolism) of admitted stroke patients after the installation of a minimal stroke unit equipped with heart rate, bloo...

Selective endovascular cooling for stroke entails brain-derived neurotrophic factor and splenic IL-10 modulation.
Abstract Stroke poses a serious health and economic burden, and the lack of treatment options necessitates a viable therapy. Hypothermia represents a promising stroke therapy, yet side effects of full-body cooling, such as pneumonia, limit its clinical application. Selective endovascular cooling (SEC), via infusion of cold saline through the intraarterial artery, represents an attractive alternative by locally cooling the brain while preserving body temperature. However, the mechanisms underlying SEC are poorly understood. Brain-derived neurotrophic factor (BDNF) is a widely recognized promotor of neuroplasticity ...

254 An Audit of the Frequency of Early Swallow Screens Performed in Patients Diagnosed with a Stroke
ConclusionIn summary while our hospital is succeeding in getting the vast majority of diagnosed strokes into our stroke unit we are not currently meeting the UK target for early swallow screening. We aim to roll out an education and training programme targeting nurses and doctors in our stroke unit regarding early swallow screening and re-audit this in 6-12 month ’s time.

Cough reflex testing in acute stroke: A survey of current UK service provision and speech and language therapist perceptions.
CONCLUSIONS & IMPLICATIONS: This survey gives valuable insight into the current practice and perceptions of SLTs in the UK working in acute stroke settings in relation to CRT. It highlights discrepancies between reported approaches and recommendations from existing guidelines and validation studies. The variation in responses indicates a need to develop a consensus statement and further research to guide practice. What this study adds What is already known on the subject CRT is gaining popularity as a screening tool for the clinical identification of SA with acute stroke patients. However, there is a lack of consensus ...

Risk factors for and impact of poststroke pneumonia in patients with acute ischemic stroke
Poststroke pneumonia (PSP) is a common complication of stroke and an important cause of death following stroke. However, the treatment of PSP remains inadequate due to severe impairment to the respiratory system by PSP. Thus, it is crucial to focus on preventing PSP to improve the prognosis of patients with stroke. This prospective single-center Cohort study aimed to investigate the risk factors for pulmonary infection following an ischemic stroke and identify whether PSP significantly influences the prognosis of patients after stroke. Altogether, 451 patients who were treated for acute ischemic stroke in the First A...

Predictors of 30-day mortality among patients with stroke admitted at a tertiary teaching hospital in Northwestern Tanzania: A prospective cohort study
ConclusionStroke is associated with a high 30-day mortality rate in Northwestern Tanzania. Concerted efforts are warranted in managing patients with stroke, with particular attention to individuals with severe strokes, ECG abnormalities, and swallowing difficulties to reduce early morbidity and mortality.
