CHA 2 DS 2 –VASc score predicts short- and long-term outcomes in patients with acute ischemic stroke treated with intravenous thrombolysis
AbstractThe CHA2DS2–VASc score is a validated tool to assess the thromboembolic risk in patients with atrial fibrillation. Pre-stroke CHA2DS2–VASc score may predict outcome in patients with acute ischemic stroke (AIS) without atrial fibrillation. The aim of this study was to investigate if the pre-stroke CHA2DS2–VASc score is able to predict short- and long-term outcomes in AIS patients treated with intravenous thrombolysis (IVT). The study group consisted of 256 consecutive patients admitted to the Udine University Hospital with AIS and underwent IVT between January 2015 to March 2017. The pre-stroke CH A2DS2–VASc...

Advances in stroke medicine.
Authors: Campbell BC Abstract In recent years, reperfusion therapies such as intravenous thrombolysis and endovascular thrombectomy for ischaemic stroke have dramatically reduced disability and revolutionised stroke management. Thrombolysis with alteplase is effective when administered to patients with potentially disabling stroke, who are not at high risk of bleeding, within 4.5 hours of the time the patient was last known to be well. Emerging evidence suggests that other thrombolytics such as tenecteplase may be even more effective. Treatment may be possible beyond 4.5 hours in patients selected using brain imagi...

The incidence of stroke in patients with early echocardiography after acute myocardial infarction
AbstractLeft ventricular (LV) thrombus formation after ST-elevation myocardial infarction (STEMI) increases the risk of stroke. In our center, most echocardiograms are performed within 2  days post-STEMI. However, LV thrombi often become visible later. We assessed the 1-year incidence of stroke in patients without LV thrombus on echocardiography performed early (1–2 days) vs. later (day ≥ 3) post-STEMI. This retrospective observational study included 416 patients with acut e STEMI. Patients with atrial fibrillation were excluded. All patients underwent echocardiography during admission. Patients with stroke withi...

Dabigatran etexilate for secondary stroke prevention: the first year experience from a multicenter short-term registry.
DISCUSSION: Our pilot data indicate that dabigatran appears to be safe for secondary stroke prevention during the first year of implementation of this therapy. However, high cost may limit the long-term treatment of AF patients with dabigatran, leading to early discontinuation. PMID: 24790645 [PubMed - as supplied by publisher]

Predictors of Poor Hospital Discharge Outcome in Acute Stroke Due To Atrial Fibrillation
ABSTRACT: Atrial fibrillation (AF) is a frequent cause of acute ischemic stroke that results in severe neurological disability and death despite treatment with intravenous thrombolysis (intravenous recombinant tissue plasminogen activator [rtPA]). We performed a retrospective review of a single-center registry of patients treated with intravenous rtPA for stroke. The purposes of this study were to compare intravenous rtPA treated patients with stroke with and without AF to examine independent predictors of poor hospital discharge outcome (in-hospital death or hospital discharge to a skilled nursing facility, long-term acut...

High blood pressure as a risk factor for incident stroke among very old people: a population-based cohort study
Introduction: High blood pressure (BP) increases the risk of stroke, but there is limited evidence from studies including very old people. The aim was to investigate risk factors for incident stroke among very old people. Methods: A prospective population-based cohort study was performed among participants aged at least 85 years in northern Sweden. The 955 participants were tested at their homes. BP was measured manually after 5-min supine rest. Incident stroke data were collected from medical charts guided by hospital registry, death records, and 5-year reassessments. Cox proportional hazards models were used. Results: ...

Impact of atrial fibrillation on in-hospital mortality of ischemic stroke patients and identification of promoting factors of atrial thrombi – Results from the German nationwide inpatient sample and a single-center retrospective cohort
Ischemic stroke is one of the leading causes of death and disability. Atrial fibrillation (AF) is a well-recognized risk factor for ischemic stroke. We aimed to investigate the impact of AF on in-hospital mortality of ischemic stroke patients and to identify parameters associated with intra-cardiac thrombogenic material. Patients were selected by screening the nationwide sample for ischemic stroke by ICD-Code (I63), stratified for AF. In this cohort, the association between in-hospital deaths and AF was investigated. In a second study, we performed a retrospective analysis of patients who underwent transesophageal echoc...

Retrospecting atrial fibrillation and stroke severity: impact onset time of acute ischemic stroke.
Abstract Irrespective of the duration of the therapeutic window, the prognosis for acute ischemic stroke is closely correlated with the time of treatment initiation. Time from symptom onset to hospital arrival, defined as the onset time, is the determining parameter. Besides the prehospital transport, whether relative factors of illness may have an impact on stroke onset time is uncertain, which were invested in this study through statistical analyses. Results show that relative factors of illness, such as history of atrial fibrillation and stroke severity, impact stroke onset time. PMID: 31321960 [PubMed - in process]
