Impact of operator characteristics on outcomes in transcatheter aortic valve replacement.
CONCLUSIONS: Primary operator specialty and other characteristics for TAVR were not associated with a difference in risk-adjusted in-hospital outcomes. Safety of Atrial Fibrillation Ablation with Isolated Surgical Aortic Valve Replacement.
CONCLUSIONS: Despite being a Class I recommendation, AF ablation concomitantly with other cardiac surgical procedures remains lower than current guideline recommendation in surgical AVR patients. Ablation for AF does not increase the 30-day operative mortality or perioperative morbidity compared to non-ablated patients, although new pacemaker requirements were higher in the AF ablated group. Right Heart Calcifying Fibrous Pseudotumor Presenting with Embolic Stroke.
We report a 43-year-old white female with a right-heart calcifying fibrous pseudotumor presenting with embolic stroke. This rare clinical-pathological entity should be included in the differential diagnosis of cardiac masses. Tissue diagnosis should be pursued. Aortic Valve Replacement in Young and Middle-Aged Adults: Current and Potential Roles of TAVR.
CONCLUSIONS: TAVR is increasingly used for adults <55 years. Given the uniformly excellent results with SAVR, including both mortality and morbidity - particularly regarding stroke, our data favor SAVR in this population, but a prospective trial is needed. Transcarotid vs Subclavian/Axillary Access for Transcatheter Aortic Valve Replacement with SAPIEN 3.
CONCLUSIONS: TC TAVR is associated with similar mortality and significant reduction in stroke compared to the subclavian/axillary approach. Cerebral Embolism Following Transcarotid TAVR:Factors Associated with Ipsilateral Ischemic Burden.
CONCLUSIONS: Carotid artery access for TAVR was associated with a higher number of cerebral ischemic lesions in the ipsilateral (vs. contralateral) cerebral hemisphere. Post-operative atrial fibrillation after cardiac surgery: a systematic review and meta-analysis.
CONCLUSIONS: This systematic review and meta-analysis identified an association between POAF and long-term mortality after cardiac surgery. More comprehensive POAF prevention and management, including more stringent follow-up for POAF recurrence after hospital discharge, may be indicated. While included studies used inconsistent definitions of POAF and variable exclusion criteria, estimates of heterogeneity are low. Differences in preoperative comorbidities such as age, ejection fraction, and obesity may not be fully accounted for in adjusted analyses. Implications of Methicillin-Resistant Staphylococcus aureus Carriage on Cardiac Surgical Outcomes.
CONCLUSIONS: MRSA carriers undergoing cardiac surgery are not at higher risk for mortality or SSI and can expect outcomes similar to those of non-carriers. A review of the complex landscape of stroke in left ventricular assist device trials.
CONCLUSIONS: Based on this review, we propose that future clinical trials: 1) utilize standardized stroke definitions and define stroke subtypes, 2) ensure that neurologists are integrated in study design and event adjudication, 3) include more thorough evaluations of stroke etiology using multimodality techniques, and 4) adopt the National Institutes of Health Stroke Scale to define stroke severity.
