A review of the complex landscape of stroke in left ventricular assist device trials.
CONCLUSIONS: Based on this review, we propose that future clinical trials: 1) utilize standardized stroke definitions and define stroke subtypes, 2) ensure that neurologists are integrated in study design and event adjudication, 3) include more thorough evaluations of stroke etiology using multimodality techniques, and 4) adopt the National Institutes of Health Stroke Scale to define stroke severity. Trends, Predictors, and Outcomes of Stroke After Surgical Aortic Valve Replacement in the United States.
CONCLUSIONS: The incidence of stroke after AVR has decreased but remains a significant cause of morbidity in medium- and high-risk patients. Superior outcomes can be achieved in medium- to high-risk patients at high-volume centers. PMID: 26611821 [PubMed - as supplied by publisher]

Early Operation in Patients With Mitral Valve Infective Endocarditis and Acute Stroke Is Safe.
CONCLUSIONS: MV surgery for IE and acute stroke can be performed early with a low risk of postoperative neurologic complications. When indicated, surgical intervention for MV IE complicated by acute stroke should not be delayed. PMID: 29132700 [PubMed - as supplied by publisher]

Stroke After Left Ventricular Assist Device Implantation: Outcomes in the Continuous-Flow Era.
CONCLUSIONS: Stroke while on CF-LVAD support was associated with significant mortality. To reduce the risk of stroke, it is essential to further elucidate risk factors, to optimize anticoagulation, and to further understand the impact of LVAD-related infections. PMID: 26070599 [PubMed - as supplied by publisher]

Development and Application of a Risk Prediction Model for In-Hospital Stroke After Transcatheter Aortic Valve Replacement - A Report from the Society of Thoracic Surgeons/American College of Cardiology Transcatheter Valve Therapy Registry.
CONCLUSIONS: A risk model for in-hospital stroke following TAVR was developed from the STS/ACC TVT Registry and used to estimate site-specific stroke performance. This model can serve as a valuable resource for quality improvement, clinical decision-making, and patient counseling. PMID: 30529671 [PubMed - as supplied by publisher]

Postoperative Blood Urea Nitrogen Is Associated With Stroke in Cardiac Surgical Patients.
CONCLUSIONS: In these analyses, we identified BUN as a marker of heightened postoperative stroke risk after cardiac surgical procedures. Postoperative risk markers may improve assessment of delayed postoperative strokes. PMID: 25683323 [PubMed - as supplied by publisher]

Anemia Is a Risk Factor of New Intraoperative Hemorrhagic Stroke During Valve Surgery for Endocarditis.
CONCLUSIONS: Intraoperative hemorrhagic stroke was not rare, and ectopic hemorrhagic stroke, associated with preoperative anemia, was more prevalent than hemorrhagic transformation of existing cerebral lesions. PMID: 25979239 [PubMed - as supplied by publisher]

Epidemiology of Stroke in Pediatric Cardiac Surgical Patients Supported With Extracorporeal Membrane Oxygenation.
CONCLUSIONS: This multicenter analysis demonstrates that pediatric cardiac surgical patients on ECMO are at high risk of stroke; younger or underweight patients and those with longer ECMO duration are at greatest risk, independent of procedural complexity. Future study is necessary to determine how anticoagulation or other clinical practices can be modified to reduce stroke incidence. PMID: 26298170 [PubMed - as supplied by publisher]

Stroke Volume Ratio Predicts Redilatation of the Right Ventricle After Pulmonary Valve Replacement.
CONCLUSIONS: The preoperative stroke volume ratio can predict redilatation of the right ventricle after pulmonary valve replacement and the degree of right ventricular myocardial fibrosis. PMID: 28347541 [PubMed - as supplied by publisher]

Long-Term Outcomes for Patients with Stroke After Coronary and Valve Surgery.
CONCLUSIONS: Postoperative stroke often leads to high mortality and poor long-term outcomes. Patients who experienced a severe stroke and underwent neurosurgical procedures also exhibited poor outcomes. Patients and their families should be informed of relatively high mortality and poor long-term outcomes before neurosurgical operation. PMID: 29501638 [PubMed - as supplied by publisher]

Role of Intraoperative Neurophysiological Monitoring in Preventing Stroke after Cardiac Surgery
CONCLUSIONS: Permanent EEG change and diabetes were significant postoperative stroke predictors in cardiac surgeries. Furthermore, overall survival out to 10-years postoperatively was significantly higher in the group without intraoperative neurophysiological monitoring changes, emphasizing its important predictive role.PMID:36634835 | DOI:10.1016/j.athoracsur.2023.01.004

Incidence of Postoperative Stroke Using the Heartstring Device in 1,380 Coronary Artery Bypass Graft Patients With Mild to Severe Atherosclerosis of the Ascending Aorta.
CONCLUSIONS: Compared with the Society of Thoracic Surgeons predicted risk for stroke, the Heartstring proximal anastomotic device can be safely used with all aortic grades. The most prominent benefit appears to be for patients with grade II disease and greater. PMID: 24726605 [PubMed - as supplied by publisher]

Left Subclavian Arterial Coverage and Stroke During Thoracic Aortic Endografting: A Systematic Review.
We report a systematic review of 63 studies comprising more than 3,000 patients. We conclude that stroke risk after TEVAR is increased by LSA coverage, and that LSA revascularization reduces stroke risk. LSA revascularization may lower the rate of posterior stroke. TEVAR for aneurysm is associated with increased stroke risk compared to TEVAR for dissection. PMID: 26588864 [PubMed - as supplied by publisher]

Predictors and Outcomes of Ischemic Stroke After Cardiac Surgery.
CONCLUSIONS: Stroke is a devastating complication following cardiac surgery, which increases operative morbidity and mortality. Stroke with LVO was associated with worse survival. However, early intervention did not impart a survival benefit. PMID: 32199830 [PubMed - as supplied by publisher]
