CHADS2 Score Predicts Postoperative Atrial Fibrillation in Patients Undergoing Elective Pulmonary Lobectomy.
CONCLUSIONS: Although multiple risk factors for PAF have been described, no easily applicable clinical model exists. Observed rate of PAF was significantly lower then the previously described 12% when CHADS2 was 0. CHADS2 can predict PAF in patients undergoing elective lobectomy and can identify patients to selectively institute prophylactic measures in patients at the greatest risk, such as patients with score of 2 or greater. Hypothermia and Selective Antegrade Cerebral Perfusion Is Safe for Arch Repair in Type A Dissection.
CONCLUSIONS: Unilateral selective antegrade cerebral perfusion with moderate hypothermic circulatory arrest remains a safe strategy for cerebral protection during emergent surgical repair of acute type A dissection and provides equivalent outcomes for both limited and extensive aortic arch reconstruction. Based on these data, unilateral selective antegrade cerebral perfusion and moderate hypothermic circulatory arrest may represent an optimal strategy for cerebral protection in this acute setting. Does Surgical Ablation Energy Source Affect Long-Term Success of the Concomitant Cox Maze Procedure?
CONCLUSIONS: Concomitant CM procedures performed with cryothermal energy alone or combined with bipolar radiofrequency ablation are safe and exceedingly effective. Aortic Atheroma Increases the Risk of Long-Term Mortality in 20,000 Patients.
CONCLUSIONS: Aortic atheromatous disease of any grade in the ascending and descending aorta is a significant long-term risk of long-term, all-cause mortality in cardiac operation patients. Risk of Intraatrial Thrombi After Thoracoscopic Ablation in Absence of Heparin and Appendage Closure.
CONCLUSIONS: Thoracoscopic ablation of AF can be associated with a risk of left atrial appendage thrombus formation and possibly also stroke. With administration of heparin during the ablation, followed by occlusion of the left atrial appendage as a part of the procedure, this risk can be effectively reduced. Bilateral Internal Mammary Artery Utilization in Diabetics: Friend or Foe?
CONCLUSIONS: In this statewide analysis, diabetics who received BIMA grafts (compared to diabetics with LIMA grafts or non-diabetics with BIMA grafts) had higher O/E ratios for composite morbidity/mortality as a result of higher O/E ratios for major complications. Cognition and Cerebral Infarction in Older Adults After Surgical Aortic Valve Replacement.
CONCLUSIONS: In high-risk, aged participants undergoing surgical AVR for aortic stenosis, post-operative cognitive dysfunction was surprisingly limited and was resolved by 1-year in most. Surgical repair of distal aortic arch aneurysm with distal extension: sternotomy versus thoracotomy.
CONCLUSIONS: The sternotomy approach showed better outcomes in terms of operative mortality, stroke, and pneumonia, as well as long-term survival compared to thoractomy. Impact of Aortic Atherosclerosis Burden on Outcomes of Surgical Aortic Valve Replacement.

Thrombocytopenia After Cardiopulmonary Bypass is Associated with Increased Morbidity and Mortality.
CONCLUSIONS: Post-operative thrombocytopenia is independently associated with post-operative mortality, AKI, infection, stroke, and prolonged ICU and hospital LOS. Serial platelet monitoring may help identify patients at higher risk of post-operative complications. Probability of Uneventful Recovery after Elective Aortic Root Replacement for Aortic Aneurysm.
CONCLUSIONS: This study provides novel information regarding the probability of uneventful recovery while confirming low in-hospital mortality and stroke rates after elective aortic root replacement for aortic aneurysm. Post-operative atrial fibrillation after cardiac surgery: a systematic review and meta-analysis.
CONCLUSIONS: This systematic review and meta-analysis identified an association between POAF and long-term mortality after cardiac surgery. More comprehensive POAF prevention and management, including more stringent follow-up for POAF recurrence after hospital discharge, may be indicated. While included studies used inconsistent definitions of POAF and variable exclusion criteria, estimates of heterogeneity are low. Differences in preoperative comorbidities such as age, ejection fraction, and obesity may not be fully accounted for in adjusted analyses. Direct versus side graft cannulation from the right axillary artery in thoracic aortic surgery.
CONCLUSIONS: Cannulating the right axillary through a vascular prosthetic graft reduces cannulation-related complications such as iatrogenic axillary artery dissection, and lowers stroke rates. Non-Vitamin K Antagonist Oral Anticoagulant vs Warfarin for Post Cardiac Surgery Atrial Fibrillation.
CONCLUSIONS: NOACs are associated with a reduction in post-operative LOS, without excess bleeding or other short-term complications, compared to warfarin. These findings support the broader use of NOACs as a safe alternative to warfarin in patients with post-cardiac surgery AF at elevated stroke risk and acceptable bleeding risk. Prosthesis selection for aortic valve replacement with concomitant coronary artery bypass graft
CONCLUSIONS: Long-term survival, overall reoperation, or stroke incidence was comparable among the two groups, while patients with a mechanical valve showed a greater likelihood of major bleeding events.
