Failure to Rescue: A New Society of Thoracic Surgeons Quality Metric for Cardiac Surgery
CONCLUSIONS: A new risk-adjusted FTR metric has been developed which complements existing STS performance measures. The metric specifically assesses institutional effectiveness of postoperative care, allowing hospitals to target quality improvement efforts.PMID:34242640 | DOI:10.1016/j.athoracsur.2021.06.025

Aortic valve repair decreases risks of VRE in AI at 10 years: a propensity score-matched analysis
CONCLUSIONS: This study demonstrated a lower long-term risk of VRE with repair compared to replacement, with low mortality and comparable durability. Further prospective randomized control trials are necessary to formally compare outcomes and determine superiority.PMID:34228974 | DOI:10.1016/j.athoracsur.2021.06.020

Routine Stent-Bridging to the Supraaortic Vessels in Aortic Arch Replacement - 10 year-experience
CONCLUSIONS: SAVSTEB is a comparatively simple, safe and efficacious technique to create the anastomosis between tetrabranched arch grafts and the supra-aortic arteries in the short and intermediate term. Bleeding from the anastomoses, kinking and scar associated stenosis are negligible; however, vertebral overstenting remains a critical technical issue.PMID:34186091 | DOI:10.1016/j.athoracsur.2021.05.074

Five-Year Outcomes with Conformable GORE ® TAG® endoprosthesis used in traumatic aortic transections
CONCLUSIONS: Five-year outcomes verify the CTAG device is a safe, effective, and durable option for BAI patients undergoing TEVAR.PMID:34126073 | DOI:10.1016/j.athoracsur.2021.05.054

Neurological complications in high-risk patients undergoing coronary artery bypass surgery
CONCLUSIONS: In patients with increased risk of perioperative stroke, aortic manipulation including the use of cardiopulmonary bypass or partial clamping for central anastomoses is associated with higher rates of postoperative neurological complications. These patients may benefit from off-pump surgery without aortic manipulation if complete revascularization can be ensured.PMID:34087237 | DOI:10.1016/j.athoracsur.2021.05.018

Direct Axillary Artery Cannulation for Type A Dissection and Impact of Dissected Innominate Artery
CONCLUSIONS: Routine direct AX cannulation strategy is safe in ATAD repair. Right AX cannulation can be used in most patients with IAD, even with a compromised true lumen, with low mortality, stroke and vascular injury rates.PMID:34052222 | DOI:10.1016/j.athoracsur.2021.05.008

Outcomes of Open v. Endovascular Repair of Descending Thoracic and Thoracoabdominal Aortic Aneurysms
CONCLUSIONS: Open repair of descending thoracic and thoracoabdominal aneurysms can achieve acceptable short-term outcomes with better intermediate-term outcomes than endovascular repair.PMID:34048754 | DOI:10.1016/j.athoracsur.2021.04.100

Recurrence of Pathologically Proven Papillary Fibroelastoma
CONCLUSIONS: Contrary to findings from previous studies, PFEs do recur after surgical excision. These findings emphasize the importance of postoperative follow-up with transesophageal echocardiography for identifying recurrent masses.PMID:34022212 | DOI:10.1016/j.athoracsur.2021.03.114

Sex Differences in Ascending Aortic and Arch Surgery: A Propensity-matched Comparison of 1153 Pairs
CONCLUSIONS: No differences were seen between the sexes in life-changing adverse outcomes after ascending aortic and arch procedures, although specific preoperative variables were associated with specific adverse events. Recognizing differences in preoperative risk factors for mortality between the sexes may facilitate targeted preoperative assessment, preparation, and counseling.PMID:33971171 | DOI:10.1016/j.athoracsur.2021.04.069
