Cognition and Cerebral Infarction in Older Adults After Surgical Aortic Valve Replacement.
CONCLUSIONS: In high-risk, aged participants undergoing surgical AVR for aortic stenosis, post-operative cognitive dysfunction was surprisingly limited and was resolved by 1-year in most. Post-operative cognitive dysfunction at 4-6 weeks was associated with more and larger acute cerebral infarcts.

Risk Prediction Model for Major Adverse Outcome in Proximal Thoracic Aortic Surgery.
CONCLUSIONS: The current study identified risk factors for major adverse outcome after proximal thoracic aortic surgery with HCA for degenerative pathology. The proposed simple, accurate model can quantify risk and facilitate physician-patient counseling prior to possible surgical intervention.

Early extubation without increased adverse events in high risk cardiac surgical patients.
CONCLUSIONS: A prudent and diligent multi-faceted early extubation protocol may be successful in high risk cardiac surgical patients without an increase in adverse outcomes. A larger study is needed in the future to confirm the finding.

Does Mitral Valve Calcium In Patients Undergoing Mitral Valve Replacement Portend Worse Survival?
CONCLUSIONS: Conservative approach to treat MAC achieves satisfactory results. Patients with MAC have significant comorbidities contributing to a worse survival though MAC in itself is not a risk factor for mortality.

Factors Associated with Prolonged Survival in Left Ventricular Assist Device Recipients.
CONCLUSIONS: Preoperative diabetes and amiodarone use were associated with poor long-term survival in LVAD recipients. Development of early complications of stroke, gastrointestinal bleeding, hemolysis, or pump thrombosis was also associated with poor long-term survival. Early diagnosis and treatment of these complications may improve survival in LVAD recipients.

Surgical outcomes in Behcet's disease patients with severe aortic regurgitation.
CONCLUSIONS: The findings suggest that mechanical root replacement combined with a low post-operative C-reactive protein level maintained through adjunctive immunomodulation therapy may lead to optimal surgical outcomes in Behcet's disease associated with severe aortic regurgitation.

Valve Sparing Root Replacement Provides Excellent Mid-Term Outcomes for Bicuspid Valve Aortopathy.
CONCLUSIONS: VSRR can be safely and effectively performed in young patients with bicuspid valve anatomy regardless of degree of pre-operative AI. Valve function is durable and the incidence of valve-related complications is low. VSRR is an attractive and potentially superior option to conventional root replacement in appropriately selected patients with bicuspid aortopathy.

Feasibility and Outcomes of TAVI using the Left Axillary Artery as Primary Access Site.
CONCLUSIONS: This is the first study reporting on outcome after TAVI using the LAA as default access. We conclude that it is highly feasible and safe with low rates of major vascular complications, bleeding and stroke.

Early Clinical Outcomes of Hybrid Arch Frozen Elephant Trunk Repair with the Thoraflex Hybrid Graft.
CONCLUSIONS: Hybrid aortic arch and FET repair with the Thoraflex hybrid graft appears to be associated with good clinical outcomes, despite being early in the learning curve with this graft. Further investigation with this device is warranted to establish its role within the variations of hybrid arch repair.

We describe our technique for totally thoracoscopic LAA exclusion with an epicardial clip device. This approach provides a safe and likely more effective alternative to LAA management than other endocardial devices.

Preoperative Venoarterial Extracorporeal Membrane Oxygenation Slashes Risk Score in Advanced Structural Heart Disease.
CONCLUSIONS: ECMO can be used as a bridge to heart valve or septal defect surgery in severely decompensated patients. Through recovery of end-organ function, ECMO may allow surgical correction of structural heart disease in patients considered inoperable or convert a salvage situation to an elective operation.

Minimally Invasive Approaches to Surgical Aortic Valve Replacement: A Meta-Analysis.
CONCLUSIONS: Minimally invasive approaches to AVR yield excellent outcomes in high volume centers. They reduce hospital stay and incidence of post-operative atrial fibrillation, and, therefore, should be considered in patients undergoing AVR. However, the operative approach should be selected according to surgeon's technical expertise and what is best for specific patient profile.
