STS-ACC TVT Registry of Transcatheter Aortic Valve Replacement.
Abstract The STS-ACC TVT Registry (Society of Thoracic Surgeons-American College of Cardiology Transcatheter Valve Therapy Registry) from 2011 to 2019 has collected data on 276,316 patients undergoing transcatheter aortic valve replacement (TAVR) at sites in all U.S. states. Volumes have increased every year, exceeding surgical aortic valve replacement in 2019 (72,991 vs. 57,626), and it is now performed in all U.S. states. TAVR now extends from extreme- to low-risk patients. This is the first presentation on 8,395 low-risk patients treated in 2019. In 2019, for the entire cohort, femoral access increased to 95.3%...

Sex-based outcomes after coronary artery bypass grafting.
CONCLUSIONS: Sex was not an independent predictor of long-term MACE after CABG in this population-based cohort study. Men had however higher long-term all-cause mortality and women higher risk of myocardial infarction. Long-term outcomes should be accounted for when considering sex as a risk factor for CABG. The Society of Thoracic Surgeons Intermacs 2020 Annual Report.
Abstract The STS-Intermacs 2020 Annual Report reviews outcomes on 25,551 patients undergoing primary isolated continuous flow left ventricular assist device (LVAD) implantation between 2010-2019. In 2019, 3,198 primary LVADs were implanted, which is the highest annual volume in Registry history. Compared with the previous era (2010-2014), patients implanted in the most recent era (2015-2019) were more likely to be African American (26.8% vs. 22.9%, P<.0001) and more likely to be bridged to durable LVAD with temporary mechanical support devices (36.8% vs 26.0%, P<.0001). In 2019, 50% of patients were INTERMAC...

Initial Clinical Experience with Mitral Valve Translocation for Secondary Mitral Regurgitation.
CONCLUSIONS: Mitral valve translocation creates a large surface of coaptation and effectively corrects FMR. Further study is needed to demonstrate the long-term durability and clinical utility of this operation. PMID: 33440174 [PubMed - as supplied by publisher]

Non-Vitamin K Antagonist Oral Anticoagulant vs Warfarin for Post Cardiac Surgery Atrial Fibrillation.
CONCLUSIONS: NOACs are associated with a reduction in post-operative LOS, without excess bleeding or other short-term complications, compared to warfarin. These findings support the broader use of NOACs as a safe alternative to warfarin in patients with post-cardiac surgery AF at elevated stroke risk and acceptable bleeding risk. Aortic Annular Enlargement in the Elderly: Short and Long-Term Outcomes in the United States.
CONCLUSIONS: These data suggest annular enlargement during SAVR is associated with increased short-term risk in a Medicare population. Survival curves crossed after three years, which may portend a benefit in select patients. However, annular enlargement is still only done in the minority of patients who are at risk for PPM. Direct versus side graft cannulation from the right axillary artery in thoracic aortic surgery.
CONCLUSIONS: Cannulating the right axillary through a vascular prosthetic graft reduces cannulation-related complications such as iatrogenic axillary artery dissection, and lowers stroke rates. To help prevent cannulation-related complications and stroke, we recommend the routine use of a side graft when cannulating the axillary artery. PMID: 33421389 [PubMed - as supplied by publisher]

The Specialized Donor Care Facility (SDCF) Model and Advances in Management of Thoracic Organ Donors.
CONCLUSIONS: Lack of high-quality prospective evidence results in conflicting practices across organ procurement organizations and best practices remain controversial. Future studies should focus on prospective, randomized investigations to evaluate donor management strategies. The SDCF model fosters a collaborative environment that encourages academic inquiry and is an ideal setting for these investigations. PMID: 33421385 [PubMed - as supplied by publisher]

Transcarotid Approach for Ascending Aortic Endovascular Repair.
We report a case of penetrating aortic ulcer in zone 0 of the ascending aorta with concern for free rupture that was treated by transcarotid endovascular stent graft. The patient was noted to be a poor candidate for open repair given comorbidities, frailty and age. She had a chronic occlusion of the right external iliac artery and stenosis of the left external iliac artery. Endovascular ascending aortic stenting was deployed successfully via right common carotid access. Patient had an uncomplicated postoperative course without evidence of stroke. Transcarotid approach is an optimal alternative access for patients undergoin...

Postoperative atrial fibrillation after cardiac surgery: a meta-analysis.
CONCLUSIONS: POAF is common after cardiac surgery and is associated with significantly higher rates of both short- and long-term stroke and mortality as well as increased hospital stay. Differences in hazard for long-term survival may be due to the underlying pathophysiological risk factors for POAF which differ by surgical procedure. Outcomes with Heparin-Induced Thrombocytopenia after Cardiac Surgery.
CONCLUSIONS: Although uncommon, HIT is a highly morbid and potentially lethal complication, which should reinforce the importance of timely recognition and treatment of this adverse outcome. PMID: 33307073 [PubMed - as supplied by publisher]

Hyponatremia: an overlooked risk factor associated with adverse outcomes after cardiac surgery.
CONCLUSIONS: Preoperative hyponatremia is relatively common and is associated with adverse short- and long-term outcomes after cardiac surgery. Preoperative hyponatremia can be used independently from standard risk factors to identify high risk patients for cardiac surgery. PMID: 33080237 [PubMed - as supplied by publisher]
