Cardiac Transplantation Using Hearts with Transient Dysfunction: Role of Takotsubo Like Phenotype Running Head: Takotsubo Like Syndrome in Donor Hearts.
CONCLUSIONS: In the largest analysis of transplanted donor hearts with transient LVSD, 1 in 4 had RWMAs on the initial TTE; but this was not associated with adverse post-transplant outcomes. Donor hearts with initial LVSD should be pursued irrespective of TTS-like hypokinesis or other RWMAs. PMID: 31816283 [PubMed - as supplied by publisher]

TAVR After Prior Mitral Valve Surgery: Results from the Transcatheter Valve Therapy Registry.
CONCLUSIONS: Patients with prior SMVR undergoing TAVR had similar 30-day outcomes, slightly higher 1-year mortality, and no increase in early PVL compared to those without previous SMVR. Prior SMVR should not preclude TAVR in appropriately selected patients. PMID: 31655043 [PubMed - as supplied by publisher]

Total endovascular repair of the aortic arch: initial experience in the Netherlands.
CONCLUSIONS: Total endovascular aortic arch repair using the Relay Branch device is technically feasible and effective in excluding aortic arch pathology. The observed stroke rate in the initial experience, however, was considerable. Although appealing, this new lesser-invasive technique should be carefully introduced, and its progress thoroughly evaluated. PMID: 31593657 [PubMed - as supplied by publisher]

Impact of Cerebral Perfusion on Outcomes of Aortic Surgery: STS Adult Cardiac Database Analysis.
CONCLUSIONS: For patients without dissection and requiring >30 minutes circulatory arrest, optimal cerebral protection strategies are deep hypothermia with either antegrade or retrograde cerebral perfusion; or moderate hypothermia with antegrade cerebral perfusion. Long-Term Single Centre Outcomes of Patients with Chronic Renal Dialysis Undergoing Cardiac Surgery.
CONCLUSIONS: Cardiac surgery in DD-CRF patients is associated with high morbidity and mortality. Interestingly, overall mortality was mainly none cardiac-related, where elderly patients or those presented with CHF had the worst life expectancy. However, subsequent kidney transplantation positively impact long-term survival in such patients. PMID: 31563486 [PubMed - as supplied by publisher]

Aortic Valve Replacement in Bioprosthetic Failure:Insights from The Society of Thoracic Surgeons National Database.
CONCLUSIONS: The number of patients undergoing SAVR for degenerated bioprosthesis is decreasing in U.S, particularly among older and high-risk patients. These trends may reflect the adoption of valve-in-valve transcatheter aortic valve replacement for degenerated bioprosthesis after its FDA approval in 2015. Surgical Treatment for Stand-Alone Atrial Fibrillation in North America.
CONCLUSIONS: Steady growth of stand-alone SA operations was noted. Procedures performed either on-pump or off-pump were relatively safe. However, intraoperative conversion was associated with significantly higher morbidity and mortality. Patient selection, improvement of surgical techniques, and long-term follow-up should be emphasized to improve decision-making and outcome. PMID: 31430460 [PubMed - as supplied by publisher]

Epiaortic Ultrasound to Prevent Stroke in Coronary Artery Bypass Grafting.
CONCLUSIONS: Avoiding aortic manipulation is associated with the lowest risk of stroke in patients undergoing CABG. When manipulation of the ascending aorta is planned, EAU is effective in guiding the surgical strategy to reduce the risk for embolic stroke in these patients. Dissection of Arch Branches Alone an Indication for Aggressive Arch Management in Type A Dissection?
CONCLUSIONS: In ATAAD, dissection of arch branches alone should not be an indication for routine zone 1/2/3 arch replacement; however, zone 1/2/3 arch replacement could be considered to prevent future reoperations in select patients. PMID: 31404544 [PubMed - as supplied by publisher]

Impact of Aortic Atherosclerosis Burden on Outcomes of Surgical Aortic Valve Replacement.
CONCLUSIONS: Moderate/severe aortic atherosclerosis was associated with an increased risk of adverse events following SAVR. Epiaortic ultrasound may serve as a useful adjunct for identifying patients who may benefit from strategies to reduce atheroembolic complications during SAVR. Increased Glucose Variability Is Associated With Major Adverse Events After Coronary Artery Bypass.
CONCLUSIONS: Increased 24-hour, but not 12-hour, postoperative GV following CABG is a predictor of poor outcomes. Preoperative HbA1c is not associated with MAEs after adjusting for postoperative mean glucose and GV. PMID: 31400320 [PubMed - as supplied by publisher]

Direct Cannulation in Minimally Invasive Cardiac surgery with limited resources.
CONCLUSIONS: In our experience, this approach is a reliable platform for a variety of MICS procedures and has resulted in low complication rates. The technique can be applied safely to both paediatric and adult population. Also, it is very cost effective as regular instruments and cannulas are used. Long-term outcomes of stand-alone Maze IV for persistent/long-standing persistent atrial fibrillation.
CONCLUSIONS: This study confirmed the safety and efficacy at long-term (7 years) of the stand-alone Maze IV surgical procedure for persistent/long-standing persistent AF. Indeed, more than 70% of the pts were in SR off class I/III AADs and off oral anticoagulation. PMID: 31325420 [PubMed - as supplied by publisher]
