Mid-term results after simultaneous carotid artery stenting and cardiac surgery.
CONCLUSIONS: In this small group of patients, the hybrid procedure was proved to be a safe and efficient way of treatment for patients with concomitant carotid and cardiac diseases. The low rate of perioperative complications and good midterm results are encouraging. PMID: 32247782 [PubMed - as supplied by publisher]

Probability of Uneventful Recovery after Elective Aortic Root Replacement for Aortic Aneurysm.
CONCLUSIONS: This study provides novel information regarding the probability of uneventful recovery while confirming low in-hospital mortality and stroke rates after elective aortic root replacement for aortic aneurysm. Importantly, uneventful recovery ensures excellent long-term survival. PMID: 32246934 [PubMed - as supplied by publisher]

Predictors and Outcomes of Ischemic Stroke After Cardiac Surgery.
CONCLUSIONS: Stroke is a devastating complication following cardiac surgery, which increases operative morbidity and mortality. Stroke with LVO was associated with worse survival. However, early intervention did not impart a survival benefit. PMID: 32199830 [PubMed - as supplied by publisher]

National Outcomes of Surgical Embolectomy for Acute Pulmonary Embolism.
CONCLUSIONS: In this contemporary, real-world study, mortality occurred in 19.8% of patients undergoing surgical embolectomy for acute pulmonary embolism. This represents a significant improvement compared to traditional outcomes and supports the role of surgery in the multidisciplinary treatment of this high-risk condition. PMID: 32199827 [PubMed - as supplied by publisher]

Direct Aortic versus Peripheral Arterial Cannulation in Surgery for Type-A Aortic Dissection.
CONCLUSIONS: In this institutional series, central arterial cannulation allowed a straightforward surgical repair of TAAD and achieved similar early outcomes to those of peripheral arterial cannulation. PMID: 32165179 [PubMed - as supplied by publisher]

Microplegia vs. 4:1 Blood Cardioplegia: Effectiveness and Cost Savings in Complex Cardiac Operations.
CONCLUSIONS: Our simplified microplegia technique offers several advantages over Buckberg cardioplegia without compromising myocardial protection or safety in complex, multi-component operations with extended aortic clamp times. PMID: 32160958 [PubMed - as supplied by publisher]

Intraoperative fluid balance and perioperative outcomes following aortic valve surgery.
CONCLUSIONS: In patients who underwent AVR for aortic stenosis, positive intraoperative fluid balance was associated with decreased odds of AKI. Patients developing AKI had increased 30- and 90-day mortality. While the overall incidence was low, increased intraoperative fluid balance was associated with MI and 30-day mortality, while increased ultrafiltration volume was associated with 30- and 90-day morality. Prospective studies are needed to better define proper intraoperative fluid management in patients undergoing cardiac surgery. PMID: 32151580 [PubMed - as supplied by publisher]

Impact of Perioperative Stroke on Mid Term Outcomes after Transcatheter Aortic Valve Replacement.
CONCLUSIONS: Stroke increases short term and mid-term mortality following TAVR. Risk prediction for neurologic events in TAVR could aid framework for patient selection, further improving outcomes. PMID: 32151578 [PubMed - as supplied by publisher]

Immediate surgery in acute type A dissection and neurologic dysfunction: Fighting the inevitable?
CONCLUSIONS: This study identified an association of preoperative neurologic status and specific stroke patterns after surgical repair of AAD. Irrespectively of timing of surgery and reperfusion strategies, preoperative neurologic dysfunction is strongly associated with impaired neurological outcome. HVAD Cannula Position and Hemocompatibility-related Adverse Events.
CONCLUSIONS: HVAD cannula coronal angle was associated with reduced right ventricular function and HRAE. Prospective studies evaluating surgical techniques to ensure optimal device positioning and its effects on HRAEs are warranted. PMID: 32035046 [PubMed - as supplied by publisher]

Five Year Outcomes from the US Pivotal Trial of Valiant Captivia Stent Graft for Blunt Aortic Injury.
CONCLUSIONS: This multicenter clinical trial describes excellent 5-year outcomes and durable exclusion of blunt thoracic aortic injury using a novel stent graft system. TEVAR with this endograft appears to be a safe and effective treatment option for patients with BTAI. PMID: 31991136 [PubMed - as supplied by publisher]

Effect of fenestration on Fontan procedure outcome: a meta-analysis and review.
CONCLUSIONS: In conclusion, the Fontan fenestration effectively reduce the pulmonary pressure and the need for prolonged pleural drainage. However, the risk of Fontan failure, early death and hospital length of stay was not modified. PMID: 31987825 [PubMed - as supplied by publisher]

Valve-Sparing Root Replacement vs Composite Valve Grafting in Aortic Root Dilation: A Meta-Analysis.
CONCLUSIONS: Aortic valve sparing appears to be safe, and associated with reduced late mortality, thromboembolism/stroke and bleeding compared with composite valve grafting. Late durability is equivalent. Aortic valve sparing should be considered in patients with favorable aortic valve morphology. PMID: 31981499 [PubMed - as supplied by publisher]

Thrombocytopenia After Cardiopulmonary Bypass is Associated with Increased Morbidity and Mortality.
CONCLUSIONS: Post-operative thrombocytopenia is independently associated with post-operative mortality, AKI, infection, stroke, and prolonged ICU and hospital LOS. Serial platelet monitoring may help identify patients at higher risk of post-operative complications. Further studies investigating strategies to reduce post-operative thrombocytopenia, including reducing CPB time, are needed. PMID: 31816284 [PubMed - as supplied by publisher]
