The venous system during pregnancy. Part 1: physiologic considerations
The confluence of anatomic and physiologic gestational changes, pre-existing and pregnancy-related comorbidities, surgery and anesthesia has significant implications for maternal hemodynamics. Genetic and hemodynamic factors alter endothelial migration and branching during embryogenesis to create separate venous and arterial systems,1 which subsequently possess distinct responses to disease processes (e.g. venous insufficiency, thrombo-embolism, stroke).2

Pharmacists Act on Care Transitions in Stroke (PACT-Stroke): A Systems Approach
This study was a single-center, retrospective assessment of patients contacted by a pharmacist. All data were analyzed descriptively. Our initial evaluation of the Pharmacists Act on Care Transitions in Stroke (PACT-Stroke) service was to quantify and categorize drug-related problems (DRPs), which included drug selection, drug form, dose selection, treatment duration, dispensing, drug use process, patient-related problems, and other. Patients were included if they were adults who experienced an ischemic stroke. Exclusion criteria for this service included pediatric patients, patients with hemorrhagic strokes, patients with...

New ERLEADA ® (apalutamide) Analysis Demonstrates Rapid, Deep Prostate-Specific Antigen (PSA) Response in Patients with Metastatic Castration-Sensitive Prostate Cancer (mCSPC)
SAN FRANCISCO, Feb. 14, 2022 – The Janssen Pharmaceutical Companies of Johnson & Johnson today announced new real-world evidence data showing the initiation of ERLEADA® (apalutamide) results in high rates of rapid and deep prostate-specific antigen (PSA) response among patients with metastatic castration-sensitive prostate cancer (mCSPC). In a separate post-hoc analysis of the registrational Phase 3 SPARTAN and TITAN studies, rapid and deep PSA responses with ERLEADA® were associated with improvement in patient-reported outcomes (PROs) related to quality of life, physical wellbeing, pain, and fatigue intensity. The...

The Diabetes-Cardiovascular Connection in Women: Understanding the Known Risks, Outcomes, and Implications for Care
AbstractPurpose of ReviewCardiovascular disease (CVD) complications constitute about 50 –70% of mortality in people with diabetes. However, there remains a persistently greater relative increase in CVD morbidity and mortality in women with diabetes than in their male counterparts. This review presents recent evidence for the risks, outcomes, and management implications for women with diabetes.Recent FindingsCompared to men, women have higher BMI and more adverse cardiovascular risk profile at time of diabetes diagnosis with greater risk for coronary heart disease, stroke, vascular dementia, and heart failure. Pregnancy-s...

Hemodynamic Assessment of Pregnant People with and without Obesity by Noninvasive Bioreactance: A Pilot Study
Conclusion Fat mass gain in the period between the first and second trimesters in addition to the hemodynamic changes due to obesity and pregnancy contribute to some degree of left ventricular diastolic dysfunction which was manifested by lower SVs. Future work should investigate the possible causative role of obesity in the cardiovascular changes identified in people with obesity. Maternal Stroke Associated With Pregnancy
This article summarizes current knowledge of the epidemiology, pathophysiology, prevention, and treatment of cerebrovascular disease in pregnant and postpartum women. RECENT FINDINGS Stroke is a leading cause of maternal morbidity and mortality, and most fatal strokes are preventable. Adaptive physiologic changes of pregnancy, including hemodynamic changes, venous stasis, hypercoagulability, and immunomodulation, contribute to increased maternal stroke risk. The highest-risk time period for maternal stroke is the immediate postpartum period. Migraine and hypertensive disorders of pregnancy, including gestational hyp...

Complications of diabetes and metrics of glycaemic management derived from continuous glucose monitoring
CONCLUSIONS: The evidence supports the association between diabetes complications and CGM-derived measures of intra-day glycaemic variability. TIR emerged as the most consistent measure, supporting its emerging role in clinical practice. More longitudinal studies and trials are required to confirm these associations, particularly for type 1 diabetes where there are limited data.PMID:35094087 | DOI:10.1210/clinem/dgac034

Cerebrovascular disease in pregnancy and postpartum
Purpose of review Cerebrovascular disorders during pregnancy and puerperium are uncommon but potentially serious entities. This review aims to provide guidance on the diagnosis and management of these diseases, according to the most recent findings. Recent findings Proteinuria is no longer a mandatory criterion for the diagnosis of preeclampsia. Favourable long-term foetal and maternal outcomes are achieved in most patients with ruptured cerebral arterial malformations during pregnancy receiving interventional treatment prior to delivery. Despite the recent recommendations, physicians still hesitate to administe...

Impact of maternal height, booking weight and gestational weight gain on cardiac adaptation in pregnancy
CONCLUSIONS: Women with greater booking weight have a pathological hemodynamic profile with high CO, PVR and MAP compared to those of lesser weight. Height is the main determinant of CO, SV, HR and PVR; weight is the main determinant of MAP and gestational weight gain is the second most important influencer for SV and PVR.
