Low serum albumin: A neglected predictor in patients with cardiovascular disease
Eur J Intern Med. 2022 May 7:S0953-6205(22)00181-9. doi: 10.1016/j.ejim.2022.05.004. Online ahead of print.ABSTRACTAlbumin, the most abundant circulating protein in blood, is an essential protein which binds and transports various drugs and substances, maintains the oncotic pressure of blood and influences the physiological function of the circulatory system. Albumin also has anti-inflammatory, antioxidant, and antithrombotic properties. Evidence supports albumin's role as a strong predictor of cardiovascular (CV) risk in several patient groups. Its protective role extends to those with coronary artery disease, heart failu...

Transcatheter aortic valve implantation-related infective endocarditis: experience from an Irish tertiary referral centre
ConclusionsThis Irish cohort of TAVI-IE exhibited a similar incidence and time to presentation compared to prior international registries; however, the 1-year mortality rate was comparatively lower. The need for rigorous dental clearance pre-TAVI and maintenance of dental health post-TAVI is underscored by the high prevalence of oral streptococcus species in this cohort.

Long ‐term outcomes of patients undergoing mechanical versus bioprosthetic aortic root replacement
ConclusionsThese findings suggest that mechanical valves for ARRs may confer a survival benefit over bioprosthetic valves. Surgeon bias was likely to account for this survival advantage.

Redo surgery for noninfective isolated mitral valve disease: Initial outcome and further follow ‐up compared to primary surgery
ConclusioniMVR for  noninfective reasons is infrequent and rare. Compared to primary surgery, redo's suffer mainly from bleeding-associated morbidity. This does not translate into prolonged hospital stay or inferior immediate or long-term outcomes. Redo mitral valve replacement can be performed at no significantly in creased surgical risk compared with first surgery and the results are particularly not limited by the surgery itself.

Etiology, clinical characteristics, and outcome of infective endocarditis: 10-year experience from a tertiary care center in Pakistan
In conclusion, our study provided valuable data on the clinical characteristics and outcomes of patients with IE in a developing country. S. aureus was the most common causative agent. Heart failure and stroke were the most common complications. The presence of prosthetic valves, history of chronic kidney disease, ischemic heart disease and previous stroke were associated with a significantly increased risk of mortality. Surgical management was not associated with improved outcomes.PMID:35347974 | DOI:10.4081/monaldi.2022.2212

Very early infective endocarditis after transcatheter aortic valve replacement
ConclusionA significant proportion of IE episodes following TAVR occurs within a few weeks following the procedure and are associated with dismal outcomes. Some baseline and TAVR procedural factors were associated with VE-IE, andStaphylococcus aureus and enterococci were the main causative pathogens. These results may help to select the more appropriate antibiotic prophylaxis in TAVR procedures and guide the initial antibiotic therapy in those cases with a clinical suspicion of IE.Graphical abstractVery early infective endocarditis after trans-catheter aortic valve replacement. VE-IE indicates very early infective endocard...

Staphylococcal endocarditis with meningitis and basal ganglia infarcts mimicking meningococcemia
We present an unusual (interesting) case of staphylococcal endocarditis with meningitis mimicking meningococcemia in a young previously healthy adult who presented with basal ganglia infarct. AbstractStaphylococcus aureus is one of the common causes of infective endocarditis (IE). IE can present with various neurological complications such as stroke, brain abscess, and meningitis, the mortality rate can be very high in such cases.

Impact of Timing of Infective Endocarditis After Transcatheter Aortic Valve Implantation on Mortality
It is well established that infective endocarditis (IE) after transcatheter aortic valve implantation (TAVI) is associated with significantly increased mortality (1-year mortality rates as high as 50%) and stroke rates.1,2 Existing studies have determined the predictors of IE after TAVI and mortality in patients with IE after TAVI.1,2 Stortecky et al3 determined that the incidence of IE after TAVI was highest in the early procedural/periprocedural period ( ≤100 days). Similar findings were shown by Mentias et al.

Recurrent Strokes as the First Presentation of Occult Pancreatic Cancer; Trousseau Syndrome: A Case Report
We present a case of a 64-year-old female with recurrent ischemic stroke of unknown cause and involving different arterial territories, who was discovered to have metastatic pancreatic cancer. The initial workup with brain imaging revealed multiple acute/subacute ischemic strokes and a negative cardiac assessment. Accordingly, she underwent a whole-body positron emission tomography-computed tomography which revealed a hypermetabolic mass lesion in the pancreaticoduodenal area. Our case indicates that cancer-associated stroke should be seriously evaluated. It demonstrates the critical nature of contemplating hidden cancer i...
