Transcatheter vs Surgical Aortic Valve Replacement in Pure Native Aortic Regurgitation
CONCLUSIONS: In Medicare patients with pure native AR, TAVR with the current commercially available transcatheter valves has comparable short-term outcomes. Although long-term outcomes were inferior to SAVR, the possibility of residual confounding, biasing long-term outcomes, given older and frailer TAVR patients, cannot be excluded.PMID:36967709 | DOI:10.1016/j.athoracsur.2022.09.016

Adding Fuel to the Fire: Infective Endocarditis and the Challenge of Cerebrovascular Complications
AbstractPurpose of ReviewInfective endocarditis is a deadly disease and made more deadly by neurologic complications. We review the cerebrovascular complications of infective endocarditis and focus our discussion on medical and surgical management.Recent FindingsWhile management of stroke in the setting of infective endocarditis differs from standard stroke treatment, mechanical thrombectomy has proven safe and successful. Optimal timing of cardiac surgery in the setting of stroke remains an area of debate, but additional observational studies continue to add more detail to the discussion.SummaryCerebrovascular complicatio...

Stroke and Noninfective Native Valvular Disease
AbstractPurpose of ReviewEmbolic stroke of undetermined source is a challenging clinical entity. While less common than atrial fibrillation and endocarditis, many noninfective heart valve lesions have been associated with stroke and may be considered as culprits for cerebral infarcts when other more common causes are excluded. This review discusses the epidemiology, pathophysiology, and management of noninfective valvular diseases that are commonly associated with stroke.Recent FindingsCalcific debris from degenerating aortic and mitral valves may embolize to the cerebral vasculature causing small- or large-vessel ischemia...

Intracranial Hemorrhage as a Marker for Mycotic Aneurysm in Patients With Infective Endocarditis
Discussion Patients with left-sided and right-sided endocarditis with PFO and ICH on CNS imaging were more likely to have mycotic aneurysms detected through DSA than similar patients without ICH. Routine DSA screening for mycotic aneurysms may not be required in patients with endocarditis undergoing evaluation for valve surgery who present without ICH on CNS imaging.

The effect of adding an open distal anastomosis to proximal aneurysm repairs in bicuspid aortopathy
ConclusionWe found no significant inter-group differences in survival, reintervention, or in-hospital complication rates, with low rates of mortality, and aortic arch-related reintervention, suggesting adding open distal anastomosis may not provide benefit in BAV patients undergoing proximal aortic aneurysm repairs.

Thrombocytopenia Following Perceval Sutureless Aortic Valve Replacement in Asian Patients
CONCLUSIONS: Thrombocytopenia was common after Perceval sutureless AVR, although its impact was not significant. Although Perceval sutureless AVR was found to be a safe and effective option, preoperative assessment of potential bleeding should be performed and the Perceval valve should not be used for patients with a high bleeding risk.PMID:36709983 | DOI:10.1253/circj.CJ-22-0587

Isolated Tricuspid Operations: Society of Thoracic Surgeons Adult Cardiac Surgery Database Analysis
CONCLUSIONS: Isolated tricuspid repair was associated with lower adjusted mortality and morbidities than replacement. Beating heart operation was associated with lower adjusted major morbidities. Pre-operative MELD scores may identify high-risk patients, and referral to higher volume centers may help improve outcomes.PMID:36696939 | DOI:10.1016/j.athoracsur.2022.12.041

Safety and Efficacy of the Transaxillary Access for Minimally Invasive Aortic Valve Surgery
Conclusions: Transaxillary MICS-AVR is at least as safe as AVR by sternotomy and can be performed in the same time frame. Its advantages are fewer transfusions and quicker postoperative recovery with a significantly shorter hospital stay. The cosmetic result and unrestricted physical abilities due to the untouched sternum and ribs are unique advantages of transaxillary access.PMID:36676784 | PMC:PMC9860976 | DOI:10.3390/medicina59010160
