Ketamine
I have an medically complex patient with severe pain and severe MDD. They were admitted to the medical hospital and in conjunction with the pain service we initiated PO ketamine. Frankly, I was blown away by the response. Rapid, immediate improvement in both pain and depression which has also allowed them to progress medically. I truly think the ketamine is having a direct effect on this patients depression and the mental status improvement isn't just mediated through reduction in pain. The... Corrections
Can anyone attest to what correctional facility psychiatry is like? All I know is that it pays more. I’m seeing job offers for 500k+, for 4 days a week. Is the job too good to be true? Clinical Sobriety
Where are people at with assessing clinical sobriety these days? I'm an inpatient psychiatrist and see this documented in ED notes periodically. I'm aware of the Hack's intoxication index, though docs at my hospital don't seem to use it. Is there a formal checklist for clinical sobriety that is widely accepted, or are docs just winging it? Seeking Input for Two Job Offer Comparison
Seeking some input since I feel conflicted between 2 job offers. Both are in the same city (VHCOL). Calling out contract/agreement issues and questions
PGY4 here about to graduate and enter the work force. Google is failing me and I feel like some things my potential employers are saying may not be true. Reaction to psychiatrists doing therapy
I’m in an employed position and I’m fortunate to be supported in doing combined treatment. I usually see around 2-3 patients per day for combined treatment. One of my colleagues told me that a psyd in our department asked her if psychiatrists were qualified and could bill for therapy. I’ve always gotten a weird feeling from this person and I have a feeling she’s somewhat threatened by the fact that I do therapy as well. Has anyone else who does combined treatment experienced this? Evaluator wrote "I need to work on my professionalism towards my educators" so can I even apply to a competitive residency?
Yup, you hear that verbatim. The second evaluator for that rotation (I got 2 evaluations for this rotation), said I was very professional toward staff and patients. Child Psych or Child Neuro/NDD for Neurodevelopmental Disorders?
I am really interested in working with kid’s with developmental disabilities and cannot decide whether I want to do it through psych or Neuro (possibly NDD program) Child Psych: residency has better work life. Will also be certified to do adult psych if I wanted to work with adults too. Nice continuity of care. I worry I will miss doing ~physical~ medicine and primary care.
