Insurance credentialing hospital privilege
I want to go into private practice in Texas and I would like to get on insurance panel. I do not currently have hospital privilege and do not work at a hospital. My credentialing person says I need to either work at a hospital or get a letter from someone who does. Don't have that currently as options. Has anyone encountered it and is there any work around to it? (Source: Student Doctor Network)

Prestige in psychiatry residencies
If you were planning to be an outpatient private practice psychiatrists and had to choose between doing residency at a home institution vs ideal geographical location vs Yale/Harvard.. Would the more "prestigious" institution matter in terms of future job prospects? (Source: Student Doctor Network)

Inpatient seclusion/restraint room
Curious if having a seclusion/restraint room is standard on psych units. I've worked on about 7 different psych units across residency and attending work and they've all had some type of seclusion or restraint room, with exception of one high acuity ward with single rooms, and patients could be secluded in their own rooms. Just curious is a seclusion room is standard part of an inpatient psych unit, or optional? (Source: Student Doctor Network)

Neuropsychiatry
I'm a current M2 at a DO school who is interested interested in Psych and Neuro. I have a great deal of experience in both and recognize the differences between the two, but have enjoyed both the subject matter and what I've seen in shadowing. I recognize I will likely figure out which one I'll be going into next year - but I was wondering if there was any way to scratch my interests in both fields? I understand that Movement and Epilepsy both see a good deal of psychiatric comorbidities -... Read more (Source: Student Doctor Network)

Geriatric Unit Dispositions
As I watch the news these days. It dawned on me, if we are culturally accepting of our elected leaders having dementia, perhaps we should expand our geriatric unit dispositions. Expedite this pipeline. Arrange family meetings, at time of admission, for families that have a little bit more resources, to consult with political campaign managers, for their viability to run for office. Just check the box "Campaign Manager Consultation" it will be right under Social Work order, Dietary, and... Read more (Source: Student Doctor Network)

Advise: insurance fraud?
So I usually hang out on the premed forums, but I got this job offer working in a psychiatric private practice. I went in for training and noticed many red flags with the psychiatrist. He charges $250 for medication management and psychotherapy to insurance (100 out of pocket) but spends less than 5 minutes with each patient. Patient files are paper but the only thing in them are printer paper that has a few scribbles on it (nothing legible, not like his actual hand writing). So my question... When do you switch antipsychotics in the inpatient setting?
Curious how long you guys trial a patient on an antipsychotic before switching course and changing meds? We are under the gun to move people of course, so I often find myself 7-8 days in and scared to change meds because we'll be starting from square one all over again. I also know that we may not get peak efficacy for 6-20 weeks out. I find myself often going to olanzapine or risperdal, then changing to the other. Occasionally abilify if there's documented positive response in the... Read more (Source: Student Doctor Network)

Any advice for high school seniors going into pre-med this fall?
I am starting undergrad in the fall of 2024. I want to be a psychiatrist someday and I will do whatever it takes to get there. The school I most want to go to at the moment is UMich. I know that will probably change once I learn more about other schools, but for now I'm making UMich my motivation for studying. Does anyone have any advice for me, going into pre-med? Is there anything I can do this summer, such as a workbook, online program, or community college course that would help me... Read more (Source: Student Doctor Network)

Inpatient psych call rate
Working at a 16 bed inpatient psych unit. Call is based on 12-hour units (7a-7p and 7p-7a). Currently paid at $150/12 hours. We take admissions 24 hours a day and have pretty high turnover (sometimes 6 discharges in one day) which leads to often 3-6 or more calls per overnight. Overnight calls are mostly patients being presented for possible admission and take several minutes but a few calls coming from on the unit re: existing patients. A lot more communication from unit staff during day... Read more (Source: Student Doctor Network)

Nursing home audit
My office got a weird call from a department of health employee who says that are doing an annual nursing home audit at a facility where a patient of mine resides. They said they had some questions about the “psychiatric services I provide to the patient.” They asked to talk to my nursing staff, not me. No release of information provided. This was earlier this week and they did not call us back even. I’m not on staff at the nursing home or anything. I essentially see the patient as if... Read more (Source: Student Doctor Network)
