Patients who no longer want services
Just double checking if a patient decides they are no longer interested in receiving services from your office and do not want medications does it still require a formal discharge letter to be sent to the patient or just documentation in the chart that this was decided per the patients request. G2211
Our billing consult for the large academic department I work for recently shared that there is a new code beginning in 2024, 'care continuity' G2211, that we can supposedly add to most if not nearly all follow up appointments, even if we're also adding 90833 as well. Basically, it seems like an add-on billing code meant to partially reimburse for the expense and labor of providing longitudinal care to patients with complex conditions. You can use it with an E&M code (i.e. 99213/4/5, but not... Read more (Source: Student Doctor Network)

Boundaries in care
I had a former colleague call me yesterday and ask me to see his wife. I’m very hesitant. What are others policy on this? (Source: Student Doctor Network)

Acamprosate vs naltrexone vs both
Hey, just wanted to start a conversation about these medicines. I tend to lean on naltrexone heavily in my practice in large part because that’s just what I’ve had the most experience with. I had someone in my office today who wants to get off alcohol but is on chronic opiates for back issues. (status post fusion) which made me consider reaching for acamprosate. I’ve written it for 5 or so people and I have never had a home run, but I have also never really had tons of negative to say about... Read more (Source: Student Doctor Network)

AI workflow design thread
What's up muppets. The desire to collaboratively jam on AI-powered workflow automations and design has awoken me from an sdn slumber. I'd read through the seminal private practice threads which are quite inspiring and useful. I've scanned recent AI thread topics and found some lively interesting commentary. But besides @SmallBird 's idea about creating behavioral activation handout for depression and maybe a few ideas about clinical uses, I didn't see too many specific use case... Residency strength vs preferred location
I interviewed at MUSC and thought that the strength and lower amount of call was really great on top of the fact that I also have family very close by. The program had everything I was looking for. However, I'm really, really tired of South Carolina because I have been dreaming of moving somewhere with the mountains/outdoors and access to skiing for years. I have done everything here there is to do and I'm sick of the place. I interviewed at the program in portland and V.C.U in richmond and... Read more (Source: Student Doctor Network)

Rad onc as a second residency?
Hey everyone. I was wondering given how rad onc has been in the match the last few years, how realistic it would be for a mid-career physician (early 40's, 10 years in practice) to get accepted at a rad onc residency program as a second residency? I was super interested in rad onc as a med student but that was at the height of rad onc's competitiveness. I was an average student at best and ended up going into psychiatry. My psychiatry career has been fine, I'm making 600ksh and have a... Read more (Source: Student Doctor Network)

Heat
It is unseasonably warm. Our hospital building is old. I don't know what witchery controls the HVAC system, but it does not switch gears easily.... And so for the last two days large parts of the hospital, including many patient units, have been sweltering. It has felt like the hospital is literally and metaphorically boiling over. Patients acting up. Staff with short tempers. To the point it feels like a genuine safety issue. I'm not looking forward to this becoming a repeated pattern... (Source: Student Doctor Network)

Billing and coding questions!
I'm an attending in an 18 bed inpatient adult psychiatric unit in the midwest. I had some questions about billing and appreciate anyone's input: 1) Do you think it's reasonable to add on 90833 therapy code to most every 99233 and 99232 follow ups in the inpatient unit? Most of my patients seem to benefit from therapy, and our length of stay is short (2-4 days) I think in part because of engaging in good therapy and med management, in addition to a strong milieu and group system, so I've... Talkiatry review
Does anyone have experience with Talkiatry? I've reviewed previous forums and not much is out there. Based on those forums, it seems base is 150k with incentives (supposedly most docs make 330-360K). I wanted to see if anyone had any other information or other telepsych companies they'd recommend. (Source: Student Doctor Network)

Food
Adams applauds NYC hospitals for feeding patients plant-based meals over meat by default in climate push Eric Adams cheered NYC hospitals for serving patients plant-based meals, saying food production/consumption is the city's third-largest source of emissions. www.foxnews.com Hahahahahaha. This is a recipe ... for disaster. When I say disaster, I mean low press ganey scores. An emerging realization for very large hospitals and health systems is that food served, and ability to get at non-routine meal times, greatly impact patient satisfacti...

question about inpatient jobs
Havent done inpatient in a while and my skills are definitely rusty compared to outpatient. Are inpatient jobs more chill compared to outpatient? Are people essentially rounding 3-4 hours then going home, and they get called if someone has a question? I guess it also depends on the unit as well. I remember when I was a resident multiple attendings would do inpatient in morning and leave by like 12 and in the state hospital, rounds were incredibly fast and patients werent even seen every day...
