Academic, federal position - a few months in

Forensic Fellowship Timeline
I'm interested in forensics but cannot figure out what the timeline is. It seems like in the past, it was more of a free for all and residents would apply in the spring of 3rd year. How to get quality supervision?
I’m several years out of residency. Working at a corporate big-box place in the center of the country.
I want to keep improving, but it’s become hard to get quality supervision at this stage in the game. My colleagues are not a realistic option for a variety of reasons.

Schizoaffective Disorder and premorbid functioning
Is recovery to premorbid function (with treatment) incompatible with schizoaffective disorder in the same way it would be with schizophrenia? I'm puzzling over whether I would be able to reject a historical diagnosis of schizoaffective disorder in favor of mood disorder with psychotic features in such a case. Factitious disorder in outpatient psychiatry
Wondering if anyone has had an outpatient in psychiatry clinic with suspected factitious disorder consisting of feigned psychological symptoms.
I have a case that has been very challenging, and there are several elements of the overall presentation and histories that raise my concern for factitious disorder. However, it is impossible to prove concretely considering the possible feigned symptoms are almost purely psychological.

Wernicke's
So, I had my first case of 100%, undoubtedly wernicke's encephalopathy this week. Damndest thing. Don't want to be too detailed and identifying but the pt had risk factors and was on PO thiamine. But they were also had been altered for more than a week. It was clearly a medical delirium, not a psychiatric illness. Salary Votes
Not a new grad by a long shot…Is 270K base plus a 30K guaranteed biannual bonus desirable compensation for a 30hrs/wk ER psych gig? Offered by an academic group and set in a county facility. High COL west coast metropolis, but gig is in a neighboring low COL, less populous county…less desirable for living (35 min drive).

The health insurance is basically useless. This means an added out-of-pocket cost (at least $1200/month).

Our C/L psychiatrist recently left on short notice.
Is anyone aware of facilities, especially other VAs, where psychiatry consults from medical floors are managed by mental health staff that are non-psychiatrists (i.e. psychiatric NP, psychiatric physician assistant, mental health pharmacist, mental health social worker, psychiatric nurse, etc.)?

Can non psychiatrists be the supervising physician overseeing IOP programs?
