Online preceptor for non-clinical ultrasounds?
A few of us independent solo or small group family docs in our area are working on adding Point of Care Ultrasound (POCUS) but are having a hard time hiring a local or online preceptor to review scans we've done so we can get enough precepted scans to satisfy our med mal carrier. We've already had courses, have the equipment (mostly Butterflys) and are piling up scans but have yet to find a preceptor to review them with us. Accused of cheating
Hey guys, I’m seeking out advice. I’ve been accused of cheating on a scantron sheet. My professor thinks that I changed the bubble sheet before returning it to her to regrade it because there were discrepancies. I am not a cheater. I have several peers and faculty members who can attest to my character. I am such a hardworking student and would never even think to cheat, especially on a scantron. Peer to peer for MRIs
I have noticed a trend over the past couple months that has me suspicious. I have had 3 lumbar MRIs and a few CT scans to R/O badness come up with a peer to peer requests before approval. Granted, this is not unusual, however for this carrier (BCBS), I typically NEVER have to go through this process. Also, these are the first requests for a P2P that I've received all year, and they have all come in the last few months of the year. Has anyone else noticed a similar trend? Is there a... Measuring Quality for ED Doc pay
Another question about ED doc pay. Any of you guys and gals at your work have, as part of your compensation package, a component tied into the nebulous concept of "quality"? Silent PE
Should we look for silent pulmonary embolism in patients with deep venous thrombosis? Anybody else find themselves scanning your DVT patients for PE more frequently after the publishing of all this literature touting the high percentage of silent PEs with concomitant DVTs? Some of these numbers are crazy. 32%-66% in some of these studies. I doubt the clinical relevance. overseas anesthesia?
has anyone ever done anesthesia overseas (UK, Scandinavia, Netherlands)? if so, is the process of maybe working overseas a few years even doable? love my current PP job right now but looking for something different maybe for a few years. i don't care about the pay cut but looking to experience something different. no kids, no SO, just alot of time on my hands. thanks. Should we get a gas?
Work at a large community hospital. Elderly female last night with obvious LVO and on CTP a huge amount of MCA territory brain at risk (almost the entire hemisphere). GCS is 11 at best. She is going for EVT. I'm with her in scanner and she starts looking a little worse, some grunting respiration and increased respiratory effort. Fibular osteomyelitis
I have several patients right now with fibular osteomyelitis that were dumped on me by ortho (ankle fracture ORIF complications) and two from wound care where chronic wounds were over the fibula for months and resulted in contiguous spread. Lab work up demonstrates elevated inflammatory markers. Advanced imaging workup usually includes CT/bone scan (if hardware present) and MRI (if hardware is not present). Combined EM/Diagnostic Radiology?
Okey, why did i thought of this combination. I work as EM resident in europe and we do not evaluate much CT scans. You can learn self of course. But my thought is - if you are double specialized with rad maybe it can give much to patient flow, you can take quick evaluations self and avoid long with times till you get rad answer. You have good ultrasonography skills and if you want to avoid a bit patient contact can work for exemple 50/50 or 60/40. Maybe it is just me who thinks that way... Combined EM/Diagnostic Radiology?
