How much is a med school acceptance worth?
Just a fun thought that crossed my mind after reading about some admissions scandals. We spend a lot of time and money to get to the point of applying. IBR? Loan advice?
I’ve scanned through and haven’t found a post that seems to fully answer my question. I have about $150,00 in student loans left, all of which are federal. When I first graduated I picked the 25 year extended payment plan. I decided not to do IBR because the payments were about the same and I assumed my income would increase. Since this will now be year 3 without a raise or bonus, does it make sense to backtrack and sign up for IBR? I picked the plan I did with the intent to pay extra and... IBR? Loan advice? Pediatric Sedation Training
Is anyone here aware of any formal training programs for pediatric sedation that last for a month or two? There is an increasing push for pediatric hospitalists to take over sedation services for minor procedures and scans, and so far the training programs they are using seems to be following the dental model (i.e. two days of training and then best of luck). TAP block with neuropathic pain
Have new referral and wanted to see if anyone has seen this. Young guy with recent resection of sigmoid tumor. Had TAP block with US prior and did well for the first week or 2. Now has odd neuropathic pain in similar distribution you would expect coverage from block. CT scan and lab work up for infection done without cause. Has anyone run into this before? What ’s working at a critical access ED like?
For those with long experience working these shops... What’s a typical shift like? How good are your nursing staff? Do you manage/round on admitted floor patients as well? Can you actually get some sleep on overnights? Do you have access to CT scanners? And as an aside, what rate did you get as locums/traveler? The place I’ll start working at sees about 1.4 patients per hour. Pretty good pay. The closest large hospital is actually only about 1.5 hour car ride, so not too bad distance wise. Mistake on the floor. Grounds for termination?
So a 89 year old female came in with a left IT fracture, not demented, AAOx3, no other injuries. She was just complaining of some minor abdominal pain and diffuse pain on the left hip area. She was assessed, scanned and labs were taken. CT all clear other than the left IT fx, labs significant for AKI, CKD and elevated troponin. Whole body screening MRI/CAT scans
[Edit- mostly talking about MRI here] Anybody else thinking of doing this once you have those (hopefully) big fat attending paychecks? I'm not going to lie, after months of inpatient medicine cancer has really began to freak me out and the thought of just passively waiting and hoping I never get it doesn't sit right with me. CT sufficient prior to Epidural?
Patient is an elderly female with h/o cardiac stent on anticoagulation also with a pacemaker that is not compatible with MRI. She has lower extremity radicular pain. Ordinarily I would like to have a MRI so that I can see soft tissue pathology. There is a CT scan which notes that there "may be some mild canal and neur foraminal narrowing." Clinically pain is only on ambulation with no associated neurologic deficits by history nor in the office on exam. Would you proceed with intervention... CT sufficient prior to Epidural? Should I go into Radiology?
