Population Pharmacokinetics of Posaconazole in Immune-Compromised Children and Assessment of Target Attainment in Invasive Fungal Disease
ConclusionsThe results of this study revealed that both fixed and weight-based adaptive dosing schemes can be appropriate for target attainment across all PSZ formulations, including suspension. Additionally, covariate analysis suggests that concomitant proton pump inhibitors should be contraindicated during PSZ suspension dosing. (Source: Clinical Pharmacokinetics)

Esophageal Plaques in a 68-Year-Old Woman With Systemic Sclerosis
A 68-year-old woman with a 20-year history of systemic sclerosis suffered from chronic iron deficiency anemia and required multiple iron infusions and packed red blood cell transfusions. Repeated upper endoscopies had shown active gastric antral vascular ectasias (GAVE), a known complication of systemic sclerosis that causes chronic gastric blood loss. She had undergone intermittent upper endoscopic ablation using argon plasma coagulation (APC) to control the persistent blood loss from GAVE. In addition, she needed pantoprazole 40 mg twice daily to control her acid reflux symptoms adequately. (Source: The American Journal of Medicine)

Determinants of severe QTc prolongation in a real-world gerontopsychiatric setting
ConclusionIn addition to the use of potentially QTc-prolonging drugs, other risk factors, especially cardiac diseases, appear to be relevant for the development of QTc prolongation in gerontopsychiatric patients. Pantoprazole was frequently involved in potential drug interactions and should generally not be used for more than 8 weeks in geriatric populations. As clinical consequences of QTc prolongation were rare, potentially QTc-prolonging drugs should not be used overcautiously; their therapeutic benefit should be considered as well. It is paramount to perform diligent benefit–risk analyses prior to the initiation of p...

Vitamin E-induced coagulopathy in a young patient: a case report
ConclusionsVitamin E-related inhibition of vitamin K-dependent factors with coagulopathy may occur even at marginally increased levels of serum vitamin E. This risk becomes significant in patients receiving other drugs that may increase the risk of bleeding. (Source: Journal of Medical Case Reports)

Gastric calcinosis ulcer in a renal transplanted patient
Rev Esp Enferm Dig. 2023 Mar 10. doi: 10.17235/reed.2023.9495/2023. Online ahead of print.ABSTRACTA 53-year-old lady with dysfunctional renal transplant and post-surgical hypoparathyroidism with phosphocalcic metabolism impairment was admitted to hospital because of long-lasting epigastric pain and nausea. An esophagogastroduodenoscopy was performed, visualising a nodular lesion of 1 cm diameter with a depressed and ulcerated base. Microscopically the lesion was in relation with a metastatic calcinosis ulcer. Pantoprazole was initiated and serum phosphocalcic levels adjusted, achieving symptom remission. In the follow-up e...
