Work for big medical group vs. start over from scratch?
Hey there SDN, long time listener . . . first time caller, I wanted to get input from the bigger brains on this. My current employer, a multi-specialty group (30 docs, 8 different specialties) that had been in existence since 1960 was run into the ground by poor management and the refusal to evolve with healthcare. Long-story short - I have been given a "red pill" vs. "blue pill" choice. I have been in this current job for 4 years, but I've been out of fellowship over 10 years. Some... SC/NC Pain Position
Hello, I am in a PP group in upstate SC with several offices in western NC that is looking for a new physician. Practice has been around for >25 years and we have very good relationships with our referral sources that keeps us busy. We have 6 physicians and 2 PhD Psychologists plus a number of PA/NP that help with follow ups. One physician is retiring which is why we are looking and I would expect whom ever was to enter our practice to get busy relatively quickly because of that. We are... Right buttock pain down the leg, need management advice
Sorry for the long write up, but this case has been bothering me. I have a 40 something healthy woman complaining of right lower buttock pain since September 2022 (no trauma, sudden onset). The pain is 4/10 at baseline but worsens to 8/10 with driving (burning sensation). There is also numbness and tingling radiating from the buttock down the lateral hip, calf, and outer sole of her right foot. Imaging: MRI Lumbar: L4-5 with 3mm disc bulge & central annular fissure, mild to moderate... Read more (Source: Student Doctor Network)

40-50 patients a day. How?
In my first year of attendinghood looking to move jobs. Group I'm considering with fairly high overhead costs. Current pain docs seeing 40-50 patients a day on clinic days with encounters 15 minutes with double bookings for procedural followup. No opioid management. In spite of asking, did not have the opportunity to shadow in clinic. I feel like this forum has led me to believe most docs seeing 20-30 a day or so. For those pushing above 40 patients daily (particularly in the 40-60... Read more (Source: Student Doctor Network)

My group is looking to hire another interventional pain doc due to volume and community need. I’m anesthesia/pain and do mostly bread and butter injections, lots of RFA, some joint injections, and some SCS, and my partner who is PM&R/spine fellowship does lumbar interventions and EMGs. Procedures almost all done in clinic including SCS trial and kypho. No opioid management (unless you want to). I don’t. hospital privileges
As pertains specifically to physicians who have an outpatient pain management practice, what are the pros and cons of getting hospital privileges? I know they are not necessary, but are they helpful? Are there any reasons not to get privileged at a local hospital? Appreciate any responses. (Source: Student Doctor Network)

FM/a fibromyalgia lab test
Just curious to know your thoughts about this? I saw my first patient who says he has been approved for the test (not ordered by me), although has not had it done yet. I was not aware of it previously. I am not sure that it would change my management of this patient. I suppose whether or not it is helpful may depend in part on how he feels about the result. Pediatrician prescribing oxycodone as a maintenance pain control rx? Out of scope?
So I encountered an rx from a pediatrician for an adult for oxy 30mg month supply. I refused to fill because it was out of scope since any high dose maintenance pain control rx should be by a pain management specialist. Was this the correct way? Thank you (Source: Student Doctor Network)
