Pediatric Pain Medicine Fellowships
Anyone have any insight into this field, it seems relatively new with only afew institutions offering the fellowship. Do you receive a general certificate as a pain physician, or are you limited to only pediatric pain management? What is the caseload? Terminal pediatric patients? Procedural heavy? Pharmacist in need of advice
A patient comes in with prescriptions for hydromorphone, alprazolam, carisoprodol (I also saw gabapentin on her profile) all from the same prescriber from a reputable pain management clinic. I told her I would not fill her scripts until I talked to her doctor/physician's assistant, which isn't possible since I'm staffed overnight. I normally refuse all holy trinity scripts, but this is the first time I've see that combo prescribed from that clinic (we fill a ton of their other scripts and... 27 y.o male was referred my to pain clinic for continued pain management (opiate therapy) and c/o continued lumbar and lower thoracic (T8-T12) pain. Pain that persist and worsens through the night and has been present and worsening/staying around the same intensity for approx. two years. The pt. proposes a few challenges towards treatment from a pain management perspective. From PCP referral paperwork...Patient takes oxycodone 15 mg TID, works at gas station. Family hx of alcoholism. Single mom. Also has fibro. How do you appraise the providers medical decision making to refer a patient who declines pain management to a pain clinic? Is this patient-centered care with justifiable shared decision making?
