Abandoned Practices for Chronic Pain???
A lawyer asked me a couple thought-provoking questions: Lawyer: "Doctor, is there a professional consensus on the use of opioids for chronic pain?" Me: "No." Lawyer: "Doctor, has the field of pain management abandoned using opioids for treating chronic pain?" Me: "No." So, it got me thinking. What have we abandoned? How will we ever know that there is a consensus about the appropriate use of a modality based upon evidence? Everything (chiro, acupuncture, opioids, PT, psych, etc)... EM Pain Fellowship vs CC? Kind of want out of EM. Any advice?
Hey guys, I'm reaching out for a little help and advice. I'm in a pretty rough place personally/emotionally and am not sure that I can do 20-30 years of EM. A little about me: I'm a pgy2 at a very good 3 year EM program. I had great board scores (245+ step 1/2), grades (3.6 H/HP/P), and was top 1/4 of my class. I'm a US MD from the standard state school. CDC guidelines
I use the CDC guidelines as a scape goat whenever patients want higher doses of narcotics...and it usually helps limiting escalating doses (even tho pts aren't happy). However, I've had some providers refer to me after the patients fail to get better even after they r on 100mmeq/day (MSIR 30mg TID) . They suggest that bc im pain management I can prescribe higher doses and that the guidelines were primarily for pcps. Any thoughts? (Source: Student Doctor Network)

pm & r vs. gas -- pain management
From previous threads I have learned that a pain fellowship in gas or pm&r is a possibility for pm&r grads. What are the differences in gas vs. pm&r fellowship training? What is the scope(procedures, day to day work,etc.)of the practice of a pm&r pain specialist vs. a gas pain specialist?
