med management follow up
If you provide opioid med management, how often are you having patients follow up? How do you handle refills (call-in, multiple scripts given at visit)? If you are providing non-opioid management (gaba, duloxetine, etc), how often for follow up? Thank you (Source: Student Doctor Network)

Question for the Pre Meds on the forum
I am invited to give a lecture at a local college. I have a lot of ideas regarding topics that they would find interesting but wanted the opinion from premed/med students here. A topic high on my list is ' Moving Beyond Opioids: The Future of Pain Management' or 'Healthy joints for life' My goal is to talk about PM&R as a specialty, anatomy/pathophysiology of joints and treatment options. Any suggestions would be greatly appreciated. (Source: Student Doctor Network)

Is the combo of Ketamine and Dexmedetomidine Hazardous?
My response is a definitive NO. These drugs are a great combo and act in a synergistic manner. I like the Combo a great deal as do many others. A Treatment Protocol for Opioid-Minimal Postoperative Pain Management A step-by-step look into how Arctic Surgery Group treats postoperative patients. www.mdmag.com But, a recent article is now claiming the combination of those 2 drugs requires additional monitoring prior to discharge. My anecdotal use shows nothing of the sort if the drugs...

New vs old pain management
I have noticed over the last several years some differences in training among new fellows emerging- 1. Very few new docs do cervical rf from a supine/foraminal approach. This is the way that the procedure was described initially and how Sluyter and Bogduk did the procedure. It is faster and has a much better effect on higher segments (which makes up the majority of cervical axial pain). I am not sure why the switch has happened, but it is very consistent. 2. Lack of use of cervical rf... New vs old pain management (Source: Student Doctor Network)

IM / Anesthesia combined residency - Practice Set up?
Hey all, I have a question I haven't been able to find an answer to. With a combined IM/Anesthesia residency program, how do graduates of these programs ultimately end up combining the two practices? Is it possible to work as an OP PCP general internist with moonlighting in the OR? Can you do OP PCP work as well as pain management, all from the same office? Do most just end up working full time anesthesia as the pay is higher? Money isn't as important to me, I like variety, and I... IM / Anesthesia combined residency - Practice Set up? Admitting cancer
Question for the attendings on this forum. What is your practice if in your workup you find evidence of a previously unknown malignancy? I'm not talking about a pulmonary nodule. I mean, hepatic mass, pancreatic mass, renal, whatever. The patient's workup is otherwise benign. Maybe they have some persistent pain in the associated area. Are you admitting these patients for further workup and management? DC to home? Of course it will sometimes depend on how likely you feel patient is to be...
