Efficacy of low carbohydrate ketogenic diet in the treatment of Type 2 Diabetes
Low carbohydrate ketogenic diet (LCKD), originally used as a treatment for childhood epilepsy is currently gaining acceptance as a nutritional therapy for obesity and type 2 diabetes. In addition, this diet has a positive effect on body weight, blood glucose level, glycosylated hemoglobin, plasma lipid profile, neurological disorders and cancer. This review focus on the therapeutic effectiveness, negative effects and the rationale of using LCKD for the treatment Type 2 diabetes. It is shown that LCKD contributes to the reduction in the intake of insulin and oral antidiabetic drugs in patients with type 2 diabetes. Furtherm...

Modulation of Immune Cell Subsets by HCV Compared to Healthy Subjects and by Antiviral Therapy in Early Virologic Response HCV Genotype 4 Compensated Liver Disease Patients
This study aimed to investigate the absolute count of certain immune cell subsets in untreated HCV-infected patients compared to healthy subjects and during pegylated interferon- α and ribavirin (PegIFN/RBV) therapy. Materials and Methods: Sixty HCV genotype 4-infected patients were treated with PegIFN/RBV therapy for 52 weeks. Efficacy was measured by the early virologic response (EVR) at post-therapy. Absolute count of mature T, T helper, T cytotoxic, T activated, Natura l Killer (NK), Natural Killer/T (NKT), B, T regulatory cells (Treg) and T helper: T cytotoxic cells ratio were longitudinally analyzed by flow cytometr...

First isolation of Candida nivariensis, an emerging fungal pathogen, in Kuwait
Conclusions: This report extends the geographic distribution of C. nivariensis to the Arabian Peninsula. MALDI-TOF MS with updated software and molecular tests are needed to correctly identify C. nivariensis. Since C. nivariensis may exhibit reduced susceptibility to antifungal agents, accurate identification and antifungal susceptibility testing is essential, particularly for isolates from sterile sites, for optimal patient management. The positive rate of pulmonary embolism by CT pulmonary angiography is high in an emergency department —even in low-risk or young patients
Objective: The clinical presentation of pulmonary embolism (PE) can be various and misleading. We analysed patients with suspicion of PE and subsequently performed computed tomography pulmonary angiography (CTPA) in an emergency department of Internal Medicine, focusing on patient groups in which PE might be underestimated in the emergency setting, such as young patients and patients with low clinical probability. Material and Methods: In 2016 and 2017, all patients receiving a CTPA for the question of PE were retrospectively evaluated for clinical (age, symptoms, vital parameters) and D-Dimers. The Wells score was calcul...

The Genetic and Clinical Significance of Fetal Hemoglobin Expression in Sickle Cell Disease
Sickle cell disease (SCD) is phenotypically heterogenic. One major genetic modifying factor is the patient ’s level of fetal hemoglobin (HbF). The latter is determined by the patient’s β-globin gene cluster haplotype and cis-acting and trans-acting single gene polymorphisms (SNP) at other distant quantitative trait loci (QTL). The Arab/India haplotype is associated with persistently high HbF levels and also a relatively mild phenotype. This haplotype carries the Xmn1 (C/T) SNP, rs 7482144 in the HBG2 locus. The major identified trans-acting QTL contain SNPs residing in the BCL11A in chromosome 2 and the HMIP locus on ...

Prognostic significance of red cell distribution width in idiopathic pulmonary fibrosis and combined pulmonary fibrosis emphysema
Objective: The red cell distribution width (RDW) test is an inexpensive and available prognostic indicator of various diseases. RDW has to date been assessed in only one study as a prognostic biomarker in patients with idiopathic pulmonary fibrosis (IPF), while the relationship between the RDW and combined pulmonary fibrosis emphysema (CPFE) has yet to be reported. Subject and Method: This single-center ambidirectional study was conducted between January 2015 and December 2018 in the Atat ürk Chest Diseases and Chest Surgery Education and Research Hospital. The baseline characteristics, laboratory results and survival st...

Management of Type 2 Diabetes: Current Strategies, Unfocussed Aspects, Challenges and Alternatives
Type-2 diabetes mellitus (T2DM) accounts for more than 90 percent cases of diabetes in adults. Resistance to insulin action is the major cause that leads to chronic hyperglycemia in diabetic patients. T2DM is the consequence of activations of multiple pathways and factors that are involved in insulin resistance and β-cells dysfunction. Also, the etiology of T2DM involves the complex interplays between genetics and environmental factors. These interplays can be efficiently governed by lifestyle modifications to achieve a better management of diabetes. The present review aims at discussing the major factors inv olved in the...

Ceramic-on-Ceramic Total Hip Arthroplasty with Large Diameter Heads: A Systematic Review
Ceramic-on-ceramic (COC) total hip arthroplasties (THAs) with large heads (#x3e;36 mm) were introduced to reduce dislocation rates and restore the hip anatomy as closely as possible to the native one. To date, the literature is scarce and fragmented; a review is desirable to point out the outcomes and the possible specific complications (noise, groin pain, and taperosis). A systematic review about large-diameter COC THAs was conducted according to the PRISMA guidelines. The PubMed and Cochrane databases were searched using the terms “large”, “big”, “head”, “hip”, and “ceramic.” The methodological qualit...

Clinical significance of serum Kallistatin and ENOX1 levels in patients with coronary heart disease
BACKGROUND: Kallistatin and ENOX1 are regulators of inflammation and oxidative stress which are typical pathological reactions in atherosclerosis. However, there is limited information of kallistatin and ENOX1 in CHD. METHODS: 50 healthy controls, 56 SAP patients and 47 ACS patients were included in this study. Kallistatin and ENOX1 in serum were measured by ELISA. Chi-square test was performed to analyze categorical data. ANOVA, Pearson correlation analysis, and multiple linear regression were performed to analyze the numerical data. Finally, ROC curve was applied to assess the diagnostic value of kallistatin in CHD. RE...

Mitochondrial Haplogroup Reveals the Genetic Basis of Diabetes Mellitus Type 2 Comorbidity in Psoriasis
Objective: Published data has shown a clear link between psoriasis (Ps) and the increasing prevalence of comorbid conditions, such as diabetes mellitus type 2 (DM2). The mitochondrial genomic haplogroup's role in the potential coexistence of psoriasis and DM2 comorbidity is the subject of this study. Material and method: Ninety-eight Kuwaiti individuals were recruited in 4 cohorts (20 healthy controls (HC), 15 with DM2, 34 with psoriasis, and 29 with psoriasis and diabetes mellitus (PsDM2). An Ion Torrent S5xL was used to sequence mitochondrial DNA (mtDNA). Chi-square test (x2) was used to assess differences in the distrib...
