Heart rate variability-measured stress and academic achievement in medical students
Conclusion: This study shows that medical students with higher stress measured by HRV have higher academic achievement, especially in written exam. Further studies are needed to reconfirm the results of this study and to assess the long-term effects of HRV measured stress on medical education. (Source: Medical Principles and Practice)

Metabolomics - the stethoscope for the 21st century
Metabolomics offers systematic identification and quantification of all metabolic products from the human body. This field could provide clinicians with new sets of diagnostic biomarkers for disease states in addition to quantifying treatment response to medications at an individualised level. This literature review aims to highlight the technology underpinning metabolic profiling, identify potential applications of metabolomics in clinical practice and discuss the translational challenges that the field faces. We searched PubMed, Medline and Embase for primary and secondary research articles regarding clinical application...

Periodization in Anterior Cruciate Ligament Rehabilitation: A Novel Framework
More than 250,000 anterior cruciate ligament (ACL) injuries occur each year in the USA, and approximately 65% of these injuries undergo reconstructive surgery. Appropriate rehabilitation after ACL reconstruction can yield predictably good outcomes, with return to previous levels of activity and high knee function. At present, periodization is used at all levels of sports training. Whether conceptualized and directed by coaches, or by athletes themselves, competitors structure their training in a cyclic fashion, enabling athletes to best realize their performance goals. In practical application, sport physical therapists us...

Pulseless Electrical Activity: Detection of Underlying Causes in a Prehospital Setting
The proportion of out-of-hospital cardiac arrests (OHCA) with pulseless electrical activity (PEA) as initial rhythm is increasing. PEA should be managed by identifying the underlying cause of the arrest and treating it accordingly. This often poses a challenge in the chaotic prehospital environment with only limited resources available. The aim of this study was to review the diagnostic tools, available in a prehospital setting, and their interpretation during cardiac arrest with PEA as initial rhythm. A systematic literature search of the Pubmed database was performed. Articles were assessed for eligibility by title, abst...

Hyperhaemolytic Syndrome in Sickle cell disease: clearing the cobwebs
Sickle cell anaemia presents with a dynamic background haemolysis and deepening anaemia. This increases the demand for transfusion if any additional strain on haemopoiesis is encountered due to any other physiological or pathological. Patients with cerebrovascular accident are placed on chronic blood transfusion; those with acute sequestration and acute chest syndrome are likewise managed with blood transfusion. These patients are prone to develop blood transfusion complications including alloimmunization and hyperhaemolytic syndrome (HHS). This term is used to describe haemolysis of both transfused and ‘own’ red cells...

Are there differences in arteriovenous fistulae created for hemodialysis between nephrologists and vascular surgeons?
Objective: Arteriovenous fistulae can be a reason for frustration among nephrologists dependent on surgeons. This is why nephrologists in many centers have begun to create arteriovenous fistulas themselves. The aim of this study was to determine possible differences and predictive parameters for arteriovenous fistula function in patients whose fistulas were created by a nephrologist or a vascular surgeon. Methods. The study was organized as a retrospective, descriptive-analytical study of interventions by nephrologists and vascular surgeons during a period of 15 years. The total number of arteriovenous fistulae created by...

The usefulness of fecal calprotectin testing in pediatric gastroenterology clinical practice
Abstract Gastrointestinal tract symptoms such as abdominal pain, constipation, diarrhea, and fever are frequent causes of visits to the pediatrician. The increasing prevalence of chronic gastrointestinal tract diseases, and the falling median age of their onset, indicate the need to identify new diagnostic methods that can differentiate inflammatory bowel diseases from other gastrointestinal tract conditions. A promising non-invasive and useful marker of intestinal inflammation is fecal calprotectin. The manuscript summarizes currently available information on the use of fecal calprotectin in the diagnosis and monitoring ...

CircHIPK3 Promotes Thyroid Cancer Tumorigenesis and Invasion through the Mirna-338-3p/RAB23 Axis
Conclusion: The downregulation of circHIPK3 significantly reduced the migration, invasion and proliferation of thyroid carcinoma. Then, we demonstrated that circHIPK3 up-regulated the expression of its targe t gene RAB23 by sponging miR-338-3p to promote the tumorigenesis and invasiveness of thyroid cancer. This study is the first to find that circHIPK3 plays the role of oncogenetic circRNA in thyroid cancer, which may provide new insights into how circRNA affects the progression of thyroid cancer. Our study also showed that circHIPK3 could be a novel biomarker for thyroid cancer. (Source: Medical Principles and Practice)

ARIA 2019 Care pathways for allergic rhinitis in Kuwait health care system
This article presents the specificity of heath care system in Kuwait in regards to management of AR, with the intention to introduce proposed ICP and adopt latest ARIA recommendations. Guidelines for ICP includes views of patients and healthcare providers and covers key areas of allergic rhinitis management. This model of guidelines support real life health care better than traditional mod-els. Aiming at improving both pharmacotherapy and allergy immunotherapy (AIT), ARIA recommendations will be locally integrated in the health care system. (Source: Medical Principles and Practice)

Elevated IL-38 Serum Levels in Newly Diagnosed Multiple Sclerosis and Systemic Sclerosis Patients
Conclusion: The higher serum levels of IL-38 in new or recently untreated cases of MS and SSc patients than in treated patients and healthy controls suggest the possible role of this cytokine in the development of these diseases or as part of a feedback loop to attenuate the inflammatory conditions in early stages of these diseases.Med Princ Pract (Source: Medical Principles and Practice)

Pentoxifylline, a drug with antiviral, anti-inflammatory and bronchodilatory effects may be a possible drug candidate for SARS-CoV-2
In December 2019, a new coronavirus, named SARS-CoV-2, has emerged from China causing pneumonia outbreaks first in the Wuhan region and have now spread worldwide but there are still no “specific drug” available. In the difficulty where new synthesized drug cannot be applied immediately to patients, “conventional drug in new use” becomes a feasible solution. Chloroquine, remdesivir, favipiravi, lopinavir, ribavirin or ritonavir have shown efficacy to inhibit coronavirus in vitro. Pentoxifylline, a drug with anti-inflammatory, immunomodulatory and bronchodilatory effects, previously showed efficacy to inhibit various...

Association of patients ’ perception of quality of healthcare received and colorectal cancer screening uptake: An analysis of two national surveys in the United States
Conclusion: Our study suggested that patients who reported receiving lower quality of healthcare servi ces were less likely to have undergone and be compliant with CRC screening recommendations. It is important to pay close attention to patient feedback surveys in order to improve healthcare delivery. (Source: Medical Principles and Practice)
