Non-invasive erections induced by perineal compression may be reflex
Hadidi et al [1] suggest a new method for induction of “natural” erections during hypospadias surgery and hypothesize that the physiology of their induced erections is based on venous compression and “milking” arterial blood from the perineum to the penis. I respectfully suggest an alternative mechanism depending on reflex erections secondary to pelvic floor stimulation. (Source: Journal of Pediatric Urology)

Penile reconstruction after complete bladder exstrophy repair
The decreased penile length in patients born with bladder exstrophy (BE) results partly from pubic symphysis diastasis and the separation of the corporal bodies. Also, intrinsic shortening of anterior corporal compartment, residual penile dorsal curvature, and postsurgical scarred skin share in creation of short penile length. Adult outcomes of urinary, sexual functions and fertility after pediatric management of Differences in Sex Development: who should be followed and how?
The management of Differences of Sex Development (DSD) has evolved considerably in recent years. The questioning of systematic early childhood treatment of DSD requires a better understanding of the outcomes of such treatments and long-term studies are therefore essential to better evaluate the prognosis of DSD. Unfortunately, limitations are numerous including the limited size of the series, the absence of standardized methodology, the evaluation of managements that no longer take place today and the absence of prospective and comparative studies. Clinical phenotyping of children with nocturnal enuresis: a key classification to improve the approach
The literature shows that nocturnal enuresis is not an isolated phenomenon of urinary loss during sleep, but encompasses a set of systemic clinical manifestations that significantly influence children's quality of life and development. However, the understanding of the clinical and physiological relationship of these systemic manifestations remains a clinical challenge. The recognition of these manifestations and their subsequent categorisation, may provide better insights into integrated clinical manifestations, facilitating the understanding of pathophysiological mechanisms, and promote increased assertiveness in the ass...

The GUDplay technique: a shift of paradigm in glans reconstruction for midshaft and penoscrotal hypospadias with moderate chordee. A proposal of a new approach
After 5 years experience with the GUD (glandular urethral disassembly) technique for distal hypospadias, we present the GUDplay technique, incorporating Thiersch-Duplay tubularization of the plate till the coronal area, disassembling the glans aggressively and refurbishing the glans. Comment to “Volume of the normal prostate gland in Polish boys aged 1–17 years: Based on transabdominal ultrasound – Prospective study”
I reviewed with great interest the study by Kowalczyk et  al. [1]. I appreciate the authors’ effort in developing nomograms for prostate size in the pediatric population. In many aspects of pediatric urology, nomograms serve as necessary and important reference tools in diagnostic and clinical decision-making processes [2]. However, we carefully need t o assess and consider the method, study population and statistical handling of data that give rise to new nomograms before uncritically applying them to our daily work routine. TIP operation is not the panacea for distal hypospadias
We read with interest the recent article by Abbas et al1. The authors tried to validate an objective method to assess the urethral plate (UP) quality for tubularised incised plate (TIP) urethroplasty using their POST score published earlier. We commend the authors for their interesting idea and scientific rigor. However, there are some important messages in this paper which we would like to highlight here. Learning from the Past: How Lessons from Hinman Syndrome Can Inform the Psychological Management of Lower Urinary Tract Dysfunction
Pediatric patients with lower urinary tract dysfunction (LUTD) experience a variety of medical and psychosocial concerns that can negatively impact their clinical management and quality of life. When initially described by Drs Hinman and Baumann, patients with a form of severe LUTD, later dubbed Hinman syndrome, were characterized as having a “general attitude of being failures.” While this mention was noteworthy as it incorporated an understanding of the interplay between psychological factors and urologic conditions, there have been delays in implementing psychological intervention as a standard of care in patients w...

Outcomes of retrograde ureteroscopy in Chinese infants and toddlers under 3 years old with ureteric stones from a single center.
In recent years, there has been a worldwide increase in the incidence of urinary stone disease in the paediatric population, mainly in older children from 12- to 17-years of age [1]. Boys are most affected in the first decade of life; however, the greatest increase in incidence has been seen in older female adolescents [2]. Within the pediatric age group, clinical presentations are varied, therefore evaluation (including imaging) and management strategies must be accommodated accordingly [1,2]. Staged laparoscopic orchiopexy of intra-abdominal testis: spermatic vessels division versus traction ? a multicentric comparative study
Staged laparoscopic management of intra-abdominal testes using pedicular section is recognized as gold standard technique, successful in 85% of cases for scrotal testicular position with less than 10% testicular atrophy. Recently, Shehata proposed a new technique without pedicular division for these testes, using spermatic vessels traction, but did not provide a comparative study of the two techniques. Surgical timing and complications, with body image, quality of life, sexual function and genital sensation in patients with congenital adrenal hyperplasia
The aim of this project was to document the long-term outcomes relating to sexual function, genital sensation, body image and quality of life, in an Australian cohort of adolescent and adult women with congenital adrenal hyperplasia(CAH) who have undergone feminising genitoplasty in infancy, childhood or adolescence. Fetal biomarkers for lower urinary tract obstruction secondary to posterior urethral valves
Today, prenatal diagnosis of congenital urogenital malformations is mostly dependent on anatomical variations found on imaging. However, these findings can mislead us in telling us when to intervene, and about post-natal prognosis. Since many findings are dependent on multiple assessments, delayed diagnosis can occur, leading to less optimal outcomes compared to early intervention. Analyses of fetal urinary biomarkers have been proposed as a method of finding biological changes that are predictive for diagnosis and prognosis in fetuses at risk of kidney disease.
