We agree that retrospective database-driven studies have inherent disadvantages such as selection bias and the inability to establish causality, as outlined within our discussion section. Yet, this study approach remains one of the best options to explore our initial research question, as a prospective cohort study or randomized control trial would be difficult in pediatric patients. Response to Letter to the Editor re Residency graduates should not be expected to perform hypospadias repair without fellowship training: results from a survey of SPU membership
We appreciate the comments and concerns of the authors. The purpose of our inquiry was to question whether residency graduates without fellowship training were being prepared to perform hypospadias surgery independently upon graduation and should the new grad and their employers expect that competency. Response to Letter to the Editor re Undescended Testis: A Roundtable Discussion Based on Clinical Scenarios – Part 1
We thank the editorial board of the Journal of Pediatric Urology and the reviewers who have put great input in order to bring out a comprehensive manuscript [1]. The initial idea behind this study was firstly, to highlight the national, international and even intercontinental differences on the management of undescended testis (UDT) and learn the mentalities of each other. A potpourri of pediatric urology
I had the honor of writing the first Potpourri article in 2004 for the first edition of the Journal of Pediatric Urology published in February 2005. In those early days it was much easier to find relevant Paediatric Urology article from other sources, it is a testament to the success of the Journal that task has become increasingly difficult as our Journal has become more successful and accumulated all of the best new papers. Commentary to “Heresy - Is There a Role for Ultrasound in Management of the Non-palpable Testicle?”
The authors report that with modern US technology  coupled with dedicated pediatric radiologist participation, a higher degree of accuracy is possible in imaging NPTs compared to previous years [1]. Starting with an US would certainly be useful in patients who are difficult to carefully examine because of obesity or lack of cooperation, and in hig h-risk patients who have had previous intraabdominal surgery. Novel Pelvic Immobilization Method Following Bladder Exstrophy Repair
We introduce a novel method of pelvic immobilization following bladder exstrophy repair involving a custom-made brace that offers adequate pelvic immobilization with the added benefits of no added operative time, easy evaluation of surgical sites, simplified wound care including sponge bathing, and overall less cumbersome management for the family given its more streamlined size. Commentary to Individualized care for patients with intersex (differences of sex development): Diagnosis and treatment of aphallia
This article represents the fifth in a series devoted to the complicated and often nuanced care of children born intersex or with variations in their sex characteristics (1). Articles that are an “n of one” are often difficult from which to make broad generalizations. This article contains a patient that many may not consider as intersex and rather having a midline defect in development, however the impact and implication on the child and future adult, family and medical team remain well within this wheelhouse. We have read the article entitled “Reducing costs of referrals for non-neonatal circumcision in Florida Medicaid population” by Hernandez et al. [1] with great interest. The purpose of the study was to find a way to reduce the cost of circumcision among low-income Americans. To reduce costs, the authors recommend circumcision as soon as possible after birth. UDT Round table editorial
Congratulations to the authors for this outstanding round table discussion on the diagnosis and treatment of undescended testis [1]. This expert panel reinforces both the European and North American guidelines on the management of undescended testis, solidifying standard practices and highlighting variable approaches (references 4 and 5 in the manuscript).
