Commentary to “The correlation between ureteric orifice morphology and primary vesicoureteral reflux grade and the impact on the effectiveness of endoscopic reflux correction”
The authors report on "the correlation between ureteric orifice morphology and primary vesicoureteral reflux grade and the impact on the effectiveness of endoscopic reflux correction". (1) The classification of ureteral orifice shape and its relation to VUR was described by Lyon in 1969 into 4 distinct but highly subjective groups (2). Since its inception there has been an effort to move away from the subjective nature of ureteral orifice morphology. In our experience, the UO appearance changes with degree of bladder filling and most UOs appear as a “horseshoe” configuration. (Source: Journal of Pediatric Urology)

The Author of the commentary criticizes the criteria of success rate, affirming that some outcomes are more related to preferences in techniques of the surgeons (REF of the commentary in press). Increase in Robot Assisted Operating Room Efficiency: A Quality Improvement Study
Pyeloplasties are time-sensitive, and the most common robot assisted intervention performed in pediatric urology. Early intervention is intended to avoid permanent loss of renal function with negative long-term effects if surgery is delayed when indicated. Commentary to A multi-institutional European comparative study of open versus robotic-assisted laparoscopic ureteral reimplantation in children with high grade (IV-V) vesicoureteral reflux
I applaud the authors on embarking on the very difficult task of comparing open (OUR) versus robotic-assisted laparoscopic ureteral reimplantation (RALUR) in children with high grade (IV-V) vesicoureteral reflux (1). Multi-institutional reviews take enormous time and coordination. The authors conclude that "the overall peri-operative and post-operative complication rates are at least equivalent to OUR, but it is associated with a faster functional recovery and time to discharge." Comparison between open and minimally invasive pyeloplasty in infants: a systematic review and meta-analysis
Ureteropelvic junction obstruction (UPJO) is the most common cause of congenital hydronephrosis. Techniques such as laparoscopic pyeloplasty (LP) have gained in popularity over recent years. Although some retrospective studies have compared minimally invasive reconstructive techniques with open surgery for treatment of UPJO in infants, results remain controversial due to the small sample size in most of these studies. Many submissions in basic science and preclinical studies are based on animal experiments. We have found that many submissions do not proceed to publication. Sometimes, this can be due to a research area that is not within the scope of our readership. However, in many situations, rejection is due to inadequate reporting of experimental detail, lack of important information (such as a hypothesis and a clear translational aspect), or failure to discuss the clinical relevance of the study. Efficacy of adjuvant dexmedetomidine on penile block for pediatric hypospadias repair
The management of postoperative pain, particularly in younger children, presents a challenge due to their limited ability to articulate the intensity of pain, lower pain threshold, and restricted availability of approved analgesics for pediatric use. Unusual inguinal mass: Ectopic seminal vesicle
A 28-year-old male was referred to our radiology department with the complaint of inguinal mass. He had this mass since its childhood but has recent discomfort. First of all ultrasound was performed and it showed tubular structures connecting with each other. Doppler Ultrasound showed no flow within the tubular mass. At first a thrombosed vascular malformation or lymphocele was considered in differential diagnosis.
