Non-invasive erections induced by perineal compression may be reflex

Hadidi et al [1] suggest a new method for induction of “natural” erections during hypospadias surgery and hypothesize that the physiology of their induced erections is based on venous compression and “milking” arterial blood from the perineum to the penis. I respectfully suggest an alternative mechanism depending on reflex erections secondary to pelvic floor stimulation.
