Commentary to A multi-institutional European comparative study of open versus robotic-assisted laparoscopic ureteral reimplantation in children with high grade (IV-V) vesicoureteral reflux
I applaud the authors on embarking on the very difficult task of comparing open (OUR) versus robotic-assisted laparoscopic ureteral reimplantation (RALUR) in children with high grade (IV-V) vesicoureteral reflux (1). Multi-institutional reviews take enormous time and coordination. The authors conclude that "the overall peri-operative and post-operative complication rates are at least equivalent to OUR, but it is associated with a faster functional recovery and time to discharge." However, this conclusion may be overstated. (Source: Journal of Pediatric Urology)

Comparison between open and minimally invasive pyeloplasty in infants: a systematic review and meta-analysis
Ureteropelvic junction obstruction (UPJO) is the most common cause of congenital hydronephrosis. Techniques such as laparoscopic pyeloplasty (LP) have gained in popularity over recent years. Although some retrospective studies have compared minimally invasive reconstructive techniques with open surgery for treatment of UPJO in infants, results remain controversial due to the small sample size in most of these studies. Indocyanine green (icg)-guided lymphatic sparing laparoscopic varicocelectomy in children and adolescents. is intratesticular injection of the dye safe? a mid-term follow-up study.
Laparoscopic Palomo varicocelectomy using indocyanine green (ICG) fluorescent lymphography (FL) is standardized technique to perform lymphatic sparing and avoid post-operative hydrocele. No data regarding the safety of intratesticular injection of ICG are currently available. (Source: Journal of Pediatric Urology)

New instructions for authors specific to studies using animal experiments
Many submissions in basic science and preclinical studies are based on animal experiments. We have found that many submissions do not proceed to publication. Sometimes, this can be due to a research area that is not within the scope of our readership. However, in many situations, rejection is due to inadequate reporting of experimental detail, lack of important information (such as a hypothesis and a clear translational aspect), or failure to discuss the clinical relevance of the study. Efficacy of adjuvant dexmedetomidine on penile block for pediatric hypospadias repair
The management of postoperative pain, particularly in younger children, presents a challenge due to their limited ability to articulate the intensity of pain, lower pain threshold, and restricted availability of approved analgesics for pediatric use. Peripheral nerve blocks may be favored over neuraxial blocks because they allow quicker mobilization after surgery. (Source: Journal of Pediatric Urology)

Safety and feasability of ureteroscopy for pediatric stone, in children under 5  Years (SFUPA 5): A French multicentric study
Ureteroscopy (URS) can be proposed as first-line therapy for the management of pelvic stones from 10 to 20  mm and for lower ureteric stones in children. However, little is known about the success and the morbidity of URS in young children. Ureteroscopic treatment may present matters in young children because of the small size of the pediatric kidney and the small size of the collecting system. (Source: Journal of Pediatric Urology)

Unusual inguinal mass: Ectopic seminal vesicle
A 28-year-old male was referred to our radiology department with the complaint of inguinal mass. He had this mass since its childhood but has recent discomfort. First of all ultrasound was performed and it showed tubular structures connecting with each other. Doppler Ultrasound showed no flow within the tubular mass. At first a thrombosed vascular malformation or lymphocele was considered in differential diagnosis. A scrotal MRI (magnetic resonance imaging) was requested by urology department for further characterization. (Source: Journal of Pediatric Urology)

Effect of Bladder Volume and Compliance on Ultrasonographic Measurement of Bladder Wall Thickness in Children with Neurogenic Bladder Dysfunction
Spina bifida is the most common cause of neurogenic bladder dysfunction in children. Measurement of bladder wall thickness (BWT) is non-invasive, low-cost, and may be complementary to urodynamics in demonstrating bladder condition. It is still unknown the wall thickness response to volume change in bladders with different compliance states. (Source: Journal of Pediatric Urology)

How we select our surgical approach for cloacal reconstruction
During cloacal reconstruction, we consider both common channel (CC) and urethral length when deciding between total urogenital mobilization (TUM) and a urogenital separation (UGS). Our concern about TUM in the case of a short urethra is resultant bladder neck incompetence, while conversion to UGS after completing the TUM dissection may result in urethral injury. Use of an interposition flap harvested from the ischiorectal fossa is another measure we use to prevent potential complications. (Source: Journal of Pediatric Urology)

Commentary on Evaluating the Requirement of Ultrasonography for Children with Their First Urinary Tract Infection
The majority of guidelines recommend RBUS after first febrile UTI despite the low yield for significant findings in most children that are screened. To this end, the authors main study goal was to apply a more selective use of RBUS after febrile UTI based on recent evidence that shows an association between the presence of non-E.coli UTI and likelihood for VUR (manuscript references 15,16, 17). The authors assessed the prevalence of undetected kidney abnormalities on RBUS after first febrile UTI with non-Ecoli pathogens or after recurrent UTI, irrespective of pathogen, and compared this approach with the standard evaluatio...

Characterizing Psychosocial Services in a Pediatric Urology Practice
Psychosocial needs, which encompass behavioral health and social determinants of health (SDOH), are important mediators of the patient experience and health outcomes. However, many practices have limited experience with systematically assessing the non-billable psychosocial services provided to patients and families. (Source: Journal of Pediatric Urology)

Kidney Transplant to Vesicostomy: a safe strategy for children with end stage renal disease and lower urinary tract anomalies
Resolution of underlying urinary tract anomalies prior to kidney transplantation in patients with end stage renal disease (ESRD) secondary to uropathy, has been historically supported under the argument that this would help prevent infectious complications and graft loss. We propose to perform earlier kidney transplantation with a transient vesicostomy, deferring resolution of the uropathy to the post-transplantation period. The aim of this study was to evaluate the outcomes of kidney transplantation in children with a vesicostomy. (Source: Journal of Pediatric Urology)
