Efficacy of novel microwave energy ‐based scissors device for laparoscopic liver resection
AbstractLaparoscopic liver resection (LLR) is now widely performed. Laparoscopic energy devices require optimal hemostasis and a clear view; however, the mist caused by laparoscopic hepatic parenchymal resection reduces the visibility of the surgical field and stresses the surgeon. Moreover, when a tumor is located near the hepatic veins or major Glissonean pedicle, surgeons require advanced techniques for securing surgical margins and preserving the vessels; however, there are limitations in fine dissection using conventional energy devices. A Postresection Perfusion Deficit in the Right Colon is an Independent Predictor of Perioperative Outcome after Major Hepatectomy
ConclusionsMajor hepatectomy causes significant reduction of splanchnic perfusion. An intraoperative posthepatectomy microcirculatory perfusion deficit of the right colon is a strong and independent predictor of clinically relevant postoperative complications after major hepatectomy. Outcome of major hepatectomy in cirrhotic patients; does surgical approach matter? A propensity score matched analysis
ConclusionsThis multicenter experience suggests potential short-term benefits of mjLLR in cirrhotic patients compared to mjOLR, without compromising long-term outcome. These findings might have interesting clinical implications for the management of patients with chronic liver disease. (Source: Journal of Hepato-Biliary-Pancreatic Surgery)

Laparoscopic intragastric approach to transgastric necrosectomy
AbstractThe laparoscopic transgastric approach to infected pancreatic necrosis is an alternative to endoscopy deemed particularly interesting in patients with large retrogastric walled-off necrosis. It allows to carry out a cystogastrostomy and an initial extensive necrosectomy, adding the possibility of future endoscopic necrosectomies if needed. Frequently this technique is performed using an anterior gastrotomy to access the gastric lumen1,2. (Source: Journal of Hepato-Biliary-Pancreatic Surgery)

Clinical liver transplant tolerance: Recent topics
AbstractMore than 50 years have passed since Starzl et al. performed the first clinical liver transplantation (LT) in 1963 (1), and it has been established as the standard treatment for end-stage liver diseases after the development of surgical procedures, organ storage and immunosuppression (IS). Currently, overall 1 year survival after LT in Japan is more than 85% according to the registry of Japan Liver Transplantation Society (2). However, as the period gets longer, the long-term adverse events of IS have been recognized, such as infections, malignancies, cardiovascular disease, metabolic disorders, and renal and other...

Stent placement using the EUS ‐PD rendezvous technique for pancreatic ductal stenosis after endoscopic papillectomy
AbstractEndoscopic papillectomy (EP) is an alternative to surgery for ampullary tumors [1-3]. Although EP is less invasive, it is associated with life-threatening adverse events [4]. Pancreatic ductal stenosis may be particularly troublesome because of the risk of acute pancreatitis if the stenosis cannot be released. Herein, we report a case of successful pancreatic stent placement using the endoscopic ultrasound (EUS)-guided pancreatic duct drainage rendezvous technique for pancreatic ductal stenosis after EP. The patient was a 51-year-old woman who presented with abdominal pain 3 weeks after EP for ampullary carcinoma (...
