‘Portal Circle’
AbstractA 51-year-old man was referred to our hospital for further evaluation of a hepatic anomaly detected by abdominal ultrasonography. His medical history was significant for a childhood operation for ventricular septal defect. Contrast-enhanced dynamic computed tomography showed an extremely rare portal venous anomaly, almost like a ‘portal circle’. (Source: Journal of Hepato-Biliary-Pancreatic Surgery)

Endoscopic selective biliary stenting and reintervention using metal stents with a slim delivery system in a patient with complexly altered digestive anatomy
AbstractA 77-year-old woman who had gallbladder carcinoma with hepatic and duodenal infiltration underwent segmental duodenectomy and gastrojejunostomy with Braun enterostomy together with cholecystectomy, right hepatectomy, and left hepaticojejunostomy with Roux-en-Y anastomosis. Three months later, the left hepatic duct was obstructed due to local recurrence of the tumor (Figure 1a). (Source: Journal of Hepato-Biliary-Pancreatic Surgery)

Randomized control trial on peri ‐operative antibiotic prophylaxis in live liver donors – Are three doses enough?
AbstractDonor safety is paramount in live donor liver transplantation (LDLT)1. Live donor hepatectomy is a major surgery with an overall morbidity of 20% and major complications rate of about 4-5%1. Infective complications are the most common morbidity in live liver donors2-4. Hepatectomy being a clean contaminated surgery along with impaired glucose tolerance, long operating time and major blood loss, leads to recommended use of prophylactic peri-operative antibiotic for decreasing peri-operative surgical site infections and other infective complications5-6. Overuse of antibiotics, not just increases the cost, but also de...

Polypoid invasive carcinoma of the gallbladder ‐Another challenging polypoid neoplasm‐
ConclusionsPolypoid invasive carcinoma shared several pathological features with ICPN, such as intraluminal polypoid pattern with papillary configurations, but PICAs were invasive adenocarcinoma with destruction of muscle layer while ICPNs were preinvasive neoplasm with occasional focal stromal invasion, thus both should be differentiated from each other. Mutant KRAS as a prognostic biomarker after hepatectomy for rectal cancer metastases: does the primary disease site matter?
ConclusionsWhile the left-sided colonic disease is frequently grouped with rectal disease, our analysis suggests that there exist fundamental biologic differences that drive disparate outcomes. Although there was a trend toward significance of KRAS mutations for patients with primary rectal cancers, it failed to achieve statistical significance. Extracellular vesicles from pancreatic ductal adenocarcinoma endoscopic ultrasound ‐fine needle aspiration samples contain a protein barcode
ConclusionsThe EV proteins obtained from EUS-FNA samples contain a PDAC-specific protein barcode. The EV proteins identified from EUS-FNA samples include promising biomarkers for the diagnosis and clinical staging of PDAC. (Source: Journal of Hepato-Biliary-Pancreatic Surgery)

Impact of using scope holder on Interventional Endoscopy (with video)
AbstractIt is necessary to hold the scope in the optimum position when performing endoscopy, especially when performing treatments such as interventional endoscopy. During interventional endoscopic ultrasound (EUS) such as EUS-guided biliary drainage, it is important to keep the scope position from the time of puncture so as to maintain a good position where the force is most easily transmitted to the devices during puncture route dilation or stent insertion.
