Importance of “muscle” and “intestine” training before major HPB surgery: A review
AbstractMajor hepato ‐pancreato‐biliary (HPB) surgery is one of the most invasive abdominal surgeries. Through the experiences of several clinical trials, including those involving patients undergoing major HPB surgery, we have recognized the importance of “muscle” and “intestine” training before surgery. Th is review article summarizes the results of our clinical trials, specifically focusing on the importance of “muscle” and “intestine”. The patients with low skeletal muscle mass or those with low functional exercise capacity showed a significantly worse postoperative course and poor long‐te rm surv...

Cholangiocyte derived carcinomas and local microbiota
AbstractTrillions of bacteria are present in the gastrointestinal tract as part of the local microbiota. Bacteria have been associated with a wide range of gastrointestinal diseases including malignant neoplasms. The association of bacteria in gastrointestinal and biliary tract carcinogenesis is supported in the paradigm ofHelicobacter pylori and intestinal ‐type gastric cancer. However, the association of bacterial species to a specific carcinoma, different from intestinal‐type gastric cancer is unresolved. The relationship of bacteria to a specific malignant neoplasm can drive clinical interventions. We review the cl...

Appraisal of 5 ‐year recurrence‐free survival after surgery in pancreatic ductal adenocarcinoma
ConclusionsMost prognosticators failed to predict the risk of recurrence in the 5  years following surgery for PDAC, and recurrence can occur even at time points up to 100 months. Therefore, cure of PDAC cannot be guaranteed by a 5‐year recurrence‐free interval, and further studies into the inherent nature of PDAC are needed to develop adequate surveillance systems which ma y lead to improvements in survival. Small ‐for‐size graft, small‐for‐size syndrome and inflow modulation in living donor liver transplantation
AbstractThe extended application of living donor liver transplantation (LDLT) has revealed the problem of graft size mismatching called “small‐for‐size syndrome (SFSS)”. The initial trials to resolve this problem involved increasing the procured graft size, from left to right, and even extending to include a right lobe graft. Clinical cases of living right lobe donations have been reported since then, drawing attention to th e risks of increasing the liver volume procured from a living donor. However, not only other modes of increasing graft volume such as auxiliary or dual liver transplantation, but also control o...

How to dissect the liver parenchyma: Excavation with CUSA
AbstractA large amount of blood flows in and out of the liver through the inflow system consisting of the portal vein and hepatic artery within the Glissonean cord and the outflow system constituted by the hepatic veins. During liver parenchymal dissection, useful methods to maintain a dry operative field are to control the inflow system with the Pringle maneuver and the outflow system by managing the central venous pressure. Additionally, mature techniques of dissecting the liver parenchyma, which can prevent injury to the blood vessels and appropriately and promptly stop bleeding, are fundamental. Similar to archaeologic...
