Salvage living donor liver transplantation for hepatocellular carcinoma recurrence after hepatectomy: Quantitative prediction using ADV score
ConclusionsRisk factors and posttransplant outcomes were similar in patients undergoing salvage and primary LDLT. ADV score is surrogate biomarker for posttransplant prognosis in salvage and primary LDLT recipients. Prognostic model incorporating ADV scores can help determine whether to perform salvage LDLT. Precision Anatomy for Minimally Invasive Hepatobiliary Pancreatic Surgery: PAM ‐HBP Surgery Project
AbstractMinimally invasive hepatobiliary pancreatic (MIS ‐HBP) surgery has made rapid progress in the last decade, even for cases requiring advanced surgery1 ‐4. Various approaches for each type of MIS ‐HBP surgery have been reported by many experts. However, the most useful approach for each surgery type has not been fully discussed. When performing MIS‐HPB, various factors such as the patient’s physique and anatomy and the tumor location and progression affect the surgical procedure. There fore, surgeons attempting MIS‐HBP may need to be aware of the characteristics of each approach so that they can handle di...

Choledochocele observed on direct peroral cholangioscopy using an ultra ‐slim endoscope
AbstractA 62 ‐year‐old man was admitted to hospital with upper abdominal pain and elevated liver enzyme levels. Computed tomography showed dilatation of the common bile duct (CBD) terminal (Figure 1a) and stones in the CBD. Endoscopic retrograde cholangiopancreatography (ERCP) showed no pancreaticobiliary ma ljunction (Figure 1b). Deep CBD cannulation was impossible despite pancreatic sphincter precutting (PSP); therefore, he was referred to our hospital for further examination. Duodenoscopy revealed a bulging cystic lesion in the second portion of the duodenum (Figure 1c). (Source: Journal of Hepato-Biliary-Pancreatic Surgery)

The impact of robotics in liver surgery: A worldwide systematic review and short ‐term outcomes meta‐analysis on 2,728 cases
ConclusionsThe advantages of RLS still need to be demonstrated although early results are promising. Advantages vs open approach are demonstrated. Compared to laparoscopic surgery, minor perioperative advantages may be observed for major resections although cost analyses are still unfavorable to the robotic approach. Current status of pediatric deceased donor liver transplantation: Lessons learned from a high ‐volume center in Japan where living donation remains predominant
ConclusionsThe recent amendment of the organ allocation system, especially the introduction of pediatric prioritization, can effectively increase the chance to obtain deceased donor grafts for pediatric DDLT in Japan. The recent refinements in donor and recipient selection and in the surgical technique of split DDLT can improve the outcomes of pediatric DDLT in Japan. Reduction of gender ‐associated M2‐like tumor‐associated macrophages in the tumor microenvironment of patients with pancreatic cancer after neoadjuvant chemoradiotherapy
ConclusionsFemale gender was an independent prognostic factor possibly due to the greater reduction in CD204+ TAM infiltration in tumors after NACRT. The beneficial effects of NACRT on TAMs ’ infiltration might be associated with gender‐dependent IRF‐5 expression. (Source: Journal of Hepato-Biliary-Pancreatic Surgery)

Regenerative medicine for the hepatobiliary system: a review
AbstractLiver transplantation, the only proven treatment for end ‐stage liver disease and acute liver failure, is hampered by the scarcity of donors. Regenerative medicine provides an alternative therapeutic approach. Tremendous efforts dedicated to liver regenerative medicine include the delivery of transplantable cells, microtissues, and bioengineered whole l ivers via tissue engineering and the maintenance of partial liver function via extracorporeal support. This brief review summarizes the current status of regenerative medicine for the hepatobiliary system. For liver regenerative medicine, the focus is on strategie...

Robotic versus open distal pancreatectomy: a multi ‐institutional matched comparison analysis
ConclusionsRDP is a safe surgical strategy for PDAC and incidence of CR ‐POPF is equivalent between RDP and ODP. RDP should be considered equivalent to ODP in terms of oncological efficacy when performed in high volume and proficient centers. (Source: Journal of Hepato-Biliary-Pancreatic Surgery)

Stent placement using dual ‐channel endoscope for biloma after EUS‐guided hepaticogastrostomy
AbstractEndoscopic ultrasound ‐guided hepaticogastrostomy (EUS‐HGS) is used for malignant biliary obstruction (MBO) when endoscopic retrograde cholangioscopy is technically challenging. Clinical practical guideline recommends self‐expandable metallic stents (SEMS) of>10 cm to prevent stent migration; however, additional endoscopic reintervention occasionally becomes difficult because of the length [1 ‐4]. Here, we present a case of biloma caused by stent migration after EUS‐HGS, and plastic stent placement was successfully performed using a dual‐channel endoscope. A 57‐year‐old man with unresectable panc...
