Author's Reply: “Bacterial infections predispose to the development of portal vein thrombosis in patients with decompensated cirrhosis”
We read with great interest the letter, by Zanetto and colleagues [1], which has validated and expanded upon our recent results [2] concerning the association between bacterial infections and portal vein thrombosis (PVT) development in cirrhotic patients. They enrolled 128 patients, followed-up for up to one year. As reported, 14% developed PVT, and, as in our study, bacterial infections were more common in patients who developed PVT (56% vs 24%, p  = 0.005), and were an independent risk factor for PVT occurrence in two models of multivariate analysis (Model 1: HR 2.99, p = 0.02; Model 2: HR 3.60, p = 0.007). A new histopathological phenomenon: Pancreatic islet cell loss in the elderly population
We observed the phenomenon of pancreatic islet cell loss (ICL) in our previous histopathological study. Multiple studies have reported that a decrease in β-cells is correlated with diabetes or chronic pancreatitis. Few studies have reported ICL in a healthy population. We thank and appreciate Dr. Masood and Dr. Khalid's interest in our work and the relevance they gave to our study [1,2]. Most of the comments regard the fact that our analysis was not addressing and considering clinical outcomes and complications which are extremely important for the management of Irritable Bowel Disease (IBD) patients. The answer lies in the definition of the current analysis. The design was that of an observational retrospective analysis based on data derived from Italian administrative databases, extracted from a subsample of Italian Local Health Units (LHUs). Liver metastasis of small cell neuroendocrine prostate carcinoma
A 67-year-old man, with a past medical history of prostate adenocarcinoma 3 years ago, was admitted for abdominal distention. Endocrine therapy was initiated since then. Seven months ago, abdominal MRI revealed hepatic mass (Fig.  1a). Next, percutane-ous liver biopsy was performed. Histopathological analysis showed small-cell neuroendocrine prostate carcinoma (SCNPC). Immunohistochemical analysis showed CD56 (+++), CgA (++), Syn (+++) (Fig. 1b and Fig. 2). PSVD has been shown to be common in patients with immune-mediated diseases, including inflammatory bowel disease (IBD).
