Biomarkers of fibrosis and portal hypertension in Fontan-associated liver disease in children and adults
Fontan-associated liver disease (FALD) refers to structural and functional changes of the liver caused by the physiology of the Fontan palliation. Currently, liver biopsy is the gold standard to assess liver fibrosis of FALD. (Source: Digestive and Liver Disease)

The present paper reflects the position of the Italian Association for Neuroendocrine Tumors (Itanet), the Italian Society of Gastroenterology (SIGE), and the Italian Society of Digestive Endoscopy (SIED) regarding the management of patients affected by gastric, duodenal, and rectal neuroendocrine neoplasms (NENs) amenable to endoscopic treatment. The key questions discussed in this paper are summarized in Table 1. Data were extracted from the MEDLINE database through searches; expert opinions and recommendations are provided in accordance with the available scientific evidence and the authors' expertise. A rapidly-growing colon polyp
A 54-year-old man underwent colonoscopy in our hospital which showed the presence of multiple polyps with a diameter in the range of 0.5 –1.0 cm in the entire colon, one of which was located in the sigmoid colon with a diameter of 0.5 cm, with a IIa type pit pattern (Fig. 1A). Polypoid growth (PG)-type tumor was considered. The patient was admitted to our hospital 7 months later due to personal reasons. During the operation, a s igmoid lesion which had grown to about 1.0 cm and exhibited a IIa+Is type pit pattern was found (Fig. 1B). (Source: Digestive and Liver Disease)

IgG4-related disease with duodenal bulb involvement
A 70-year-old previously healthy male patient presented with epigastric pain persisting for about one month. Laboratory findings suggested a mild increased CA199 (82.1  U/ml, normal value 0–27 U/ml) and CA50 (48 U/ml, normal value 0–25 U/ml). Abdominal computed tomography (CT) showed diffuse pancreatic enlargement and multiple small nodules on both kidneys (Fig. 1A). PET-MR was performed and IgG4-related disease (IgG4-RD) was suspected. IgG4 was further t ested and with an elevated value (3.17 g/L, normal value 0.08–1.4 g/L). (Source: Digestive and Liver Disease)

Bile reflux is associated with Barrett's esophagus
We read with great interest the article by Iwaya et  al. [1] published by Digestive and Liver Disease. A large-scale study was conducted on the association between bile reflux and Barrett's esophagus. The results indicated that bile reflux is independently associated with the presence of Barrett's esophagus. However, a cohort study by Braghetto et  al. [2] suggested that bile reflux was not significantly associated with Barrett's esophagus and further studies were needed to confirm this hypothesis. (Source: Digestive and Liver Disease)

Author's Reply: “Immunomodulator combination versus mono therapy: Does combination therapy have an impact on infliximab therapy?”
We have read with interest the comments on our article [1] by Yuksel et  al. [2] and we thank them for their valuable insight. The authors highlighted the main conclusion of our study: there were no differences between the combination treatment group (thiopurines with infliximab) and infliximab monotherapy group regarding the occurrence of a composite outcome used to d efine disease progression. Enhanced recovery after surgery (ERAS) in advanced therapeutic flexible endoscopy: Introducing the concept of enhanced recovery after therapeutic endoscopy (ERATE)
Enhanced recovery after surgery (ERAS) guidelines are evidence-based recommendations designed to improve patient outcomes and reduce complications after surgery. Although the ERAS guidelines focus primarily on surgical procedures, many of the principles could be applied also to therapeutic endoscopy as well. An extensive literature research on Embase and PubMed was thus made to reviewed articles regarding ERAS protocols applied to therapeutic endoscopy, organized by specific endoscopic field. Out of 214 papers, only 6 were relevant to the topic. (Source: Digestive and Liver Disease)

A neurofibromatosis patient with a large appendix and multiple small intestine masses
A 53-year-old woman with a history of neurofibromatosis type 1 (NF1) presented with hematemesis. Physical examination revealed multiple neurofibromas and caf é-au-lait spots on the skin. Laboratory tests results indicated moderate anemia. Contrast-enhanced abdominal computed tomography revealed multiple lesions with soft-tissue density in the wall of the small intestine, with significant enhancement after injection of contrast agent (Fig. 1A), and an e nlarged, fluid-filled appendix with wall thickening (Fig. 1B). Role of endoscopy in hepatology
The growing and evolving field of EUS and advanced hepatobiliary endoscopy has amplified traditional upper gastrointestinal endoscopy and unveiled novel options for remaining unsolved hepatobiliary issues, both diagnostically and therapeutically. This conceptually appealing and fascinating integration of endoscopy within the practice of hepatology is referred to as ‘endo-hepatology’. Endo-hepatology focuses on the one hand on disorders of the liver parenchyma and liver vasculature and of the hepatobiliary tract on the other hand. (Source: Digestive and Liver Disease)

TREM1 facilitates the development of gastric cancer through regulating neutrophil extracellular traps-mediated macrophage polarization
Triggering receptor expressed on myeloid cell 1 (TREM1) elevation is associated with the unfavorable prognosis of gastric cancer (GC) patients. This work uncovered the effects and mechanism of TREM1 in GC. IHC staining examined TREM1 expression in GC tissues. TREM1-knockout and TREM1 knock-in mice were generated prior to the construction of N-methyl-N'-nitro-N-nitrosoguanidine (MNNG)-induced GC mice model. H&E staining detected the pathological alternations of gastric tissues. IHC staining tested Ki67 expression. (Source: Digestive and Liver Disease)

Histological evaluation of pyrrolizidine alkaloid-induced hepatic sinusoidal obstruction syndrome: Correlation with Drum Tower Severity Scoring
Hepatic sinusoidal obstruction syndrome induced by pyrrolizidine alkaloids (PA-HSOS) is a complication of drug-induced liver damage. Few studies have examined the relationship between pathological changes and clinical circumstances in PA-HSOS. The Drum Tower Severity Scoring System (DTSS) was developed using prognostic indicators from clinical treatment outcomes. We hypothesized that the severity of pathological damage is consistent with DTSS. (Source: Digestive and Liver Disease)

Exploring occupational toxicant exposures in patients with metabolic dysfunction-associated steatotic liver disease: A prospective pilot study
Metabolic dysfunction-associated steatotic liver disease (MASLD) has been traditionally associated with insulin resistance and obesity. Recently, pollutants have been shown to contribute to the development of MASLD. Given the global burden of MASLD, understanding whether pollutants are merely associated with steatosis or contribute to its progression to advanced chronic liver disease (ACLD) and hepatocellular carcinoma (HCC) is critical. Workers exposed to occupational toxicants represent an ideal population for assessing the potentially hazardous consequences of professional exposure. (Source: Digestive and Liver Disease)

Conversion surgery for initially unresectable hepatocellular carcinoma using lenvatinib combined with TACE plus PD-1 inhibitor: A real-world observational study
Conversion therapy for initially unresectable hepatocellular carcinoma (iuHCC) using lenvatinib combined with transcatheter arterial chemoembolization (TACE) plus a PD-1 inhibitor (LTP) has achieved promising results. However, further comparative research is necessary to evaluate the effectiveness and safety of  conversion surgery (CS) for iuHCC. (Source: Digestive and Liver Disease)
