Reflections on the state of diabetes research and prospects for treatment
AbstractResearch on the etiology and treatment of diabetes has made substantial progress. As a result, several new classes of anti-diabetic drugs have been introduced in clinical practice. Nonetheless, the number of patients achieving glycemic control targets has not increased for the past 20 years. Two areas of unmet medical need are the restoration of insulin sensitivity and the reversal of pancreatic beta cell failure. In this review, we integrate research advances in transcriptional regulation of insulin action and pathophysiology of beta cell dedifferentiation with their potential impact on prospects of a durable “c...

The relationship between self-care activities, social support, and glycemic control in primary healthcare patients with type 2 diabetes
ConclusionDiabetes self-care activities were linked with reduced HBA1c levels. Further studies are needed to evaluate the buffering effect of social support on glycemic outcomes in PHC patients with T2DM. (Source: Diabetology International)

A patient with ketosis-prone type 2 diabetes showing nearly normalized glucose tolerance after recovery from severe diabetic ketoacidosis
We present a case of KPD with nearly normalized glucose tolerance after recovery from severe DKA. A 41-year-old obese woman first presented with unprovoked severe DKA, i.e., ketonuria, plasma glucose 570 mg/dL, pH 7.18, an d HCO3− 5.2  mmol/L, without anti-islet autoantibodies. She achieved insulin-free glycemic remission after recovery from DKA, leading to the diagnosis of KPD. Thereafter, 75 g oral glucose tolerance test showed impaired fasting glucose and time-in-range using intermittently scanned continuous glucose monitori ng was 97% without medication. These findings suggest that, despite the initial severe DKA,...

Cases of fulminant type 1 and type 2 diabetes mellitus whose HbA1c levels were unmeasurable due to increased labile HbA1c
AbstractAlthough the measurement of hemoglobin A1c (HbA1c) using high-performance liquid chromatography (HPLC) is routinely used to estimate average blood glucose levels, it may not be accurately measured for various reasons, such as alteration of red blood cell lifespan and the existence of hemoglobin variants; including hemoglobin F (HbF). Here, we report cases of fulminant type 1 and type 2 diabetes mellitus in which HbA1c levels were unmeasurable because of increased labile HbA1c levels. Case 1 involved a 73-year-old man with fulminant type 1 diabetes mellitus, who was brought to our hospital with diabetic ketoacidosis...

Relationship between diabetic complications and the nutritional index in untreated diabetes
This study aimed to determine the association between diabetic complications and the nutritional index at the first hospital visit in untreated patients with diabetes. Two hundred and four patients with untreated type 2 diabetes were enrolled in the present study. Nutrition-related risks were assessed using the Geriatric Nutritional Risk Index (GNRI). The patients were divided into the following three subgroups: major/moderate risk, low risk, and no risk. Intergroup comparisons of clinical characteristics were carried out. The risk of complications related to diabetes was associated with the GNRI. The major/moderate-risk g...

Effect of dulaglutide and long-acting insulin combination therapy in patients with type 2 diabetes: a retrospective observational study
ConclusionCombination therapy with dulaglutide and long-acting insulin resulted in better blood glucose control than intensive insulin therapy, which persisted for 24  weeks. Combination therapy also reduced blood glucose fluctuations and the number of self-injections needed. (Source: Diabetology International)

Clinical significance of tumor markers in patients with type 2 diabetes: a retrospective observational study
ConclusionsPPV of serum CEA and CA19-9 for detecting malignancy was high in type 2 diabetes patients with poor glycemic control. Measuring CA19-9 was found to be valuable to cross-sectionally and longitudinally detect malignancies. (Source: Diabetology International)

Periodontal diseases assessed by average bone resorption are associated with microvascular complications in patients with type 2 diabetes
In conclusion, periodontal disease, evaluated by average bone resorption rate, was associated with diabetic nephropathy and neurop athy. (Source: Diabetology International)

Documented contraceptive use after delivery in women with pre-gestational and gestational diabetes in Thailand: a single center study
ConclusionsWomen with DM-complicated pregnancy frequently used contraception after delivery. However, the rate of modern contraceptive use among non-sterilized women was lower in those with PDM and the chosen methods were still inappropriate. Our findings highlight potential opportunities to improve contraceptive care and the need to promote the use of highly effective methods, seeking to minimize barriers for modern reversible contraceptive accessing. Decreased cognitive function is associated with preceding severe hypoglycemia and impaired blood glucose control in the elderly individuals with type 1 diabetes
ConclusionsIn elderly individuals with type 1 diabetes, our results suggest that high HbA1c for the past 5  years from the cognitive function test and a history of severe hypoglycemic episodes from the time of disease diagnosis are related to decreased cognitive function. Status of glycemic excursions and hypoglycemia in patients with type 1 diabetes 2  months after first initiation of intermittently scanned continuous glucose monitoring: a pilot study
ConclusionsIn T1D patients with a median HbA1c of 7.6%, HbA1c was significantly decreased 2  months after initiating isCGM, while hyT increased, particularly during daytime. Study results suggest that a reduction in the total insulin dose of about 0.10 U/kg may be required in some cases. These findings need to be taken into account when initiating isCGM. Financial burden for families of children with type 1 diabetes: a cross-sectional survey from North India
ConclusionsThere is a high financial burden of T1D care for North Indian children, almost on the verge of losing sustainability. Further studies are warranted to furnish larger OOPE data to guide policy decisions aimed at reducing direct costs to patients. (Source: Diabetology International)

High-frequency HbA1c testing among older patients with diabetes in Japan: a longitudinal analysis using medical claims data
ConclusionsWe revealed a high rate of HbA1c testing among older Japanese patients with type 2 diabetes in ambulatory care and indicated that high-frequency HbA1c testing was more likely to result from the decentralized healthcare system and its structure. (Source: Diabetology International)
