Effectiveness and Safety of Advanced Audiology-Led Triage in Pediatric Otolaryngology Services
Conclusions: These findings indicate that an Advanced Audiology-led service provides a safe and effective triaging model for the independent management of children not requiring treatment, and children requiring routine postoperative grommet review, and improves the effective use of specialist resource compared with the standard medical ORL service. Acute mastoiditis in infants aged six months or younger.
CONCLUSION: Acute mastoiditis in infants aged six months or less can be safely diagnosed and treated using a standardised algorithmic approach, similar to that used for older children. PMID: 32787992 [PubMed - as supplied by publisher] (Source: Journal of Laryngology and Otology)

Diagnosis of Stapedial Myoclonus Using Endoscopic Visualization
Conclusion: Stapedial myoclonus was diagnosed by transtympanic otoendoscopy in an awake patient. This approach may be readily applied in awake patients suspected of having stapedial myoclonus. Transection of the stapedial tendon in these patients resolves tinnitus. (Source: Otology and Neurotology)

The National Landscape of Acute Mastoiditis: Analysis of the Nationwide Readmissions Database
Conclusions: Readmission, prolonged length of stay, and discharge to a rehabilitation facility are common in patients with acute mastoiditis with various sociodemographic and disease-related risk factors. While once a primarily surgical disease, a minority of patients in our cohort underwent procedures. Undergoing a surgical procedure was protective against readmission but a risk factor for prolonged length of stay. (Source: Otology and Neurotology)

Minimally invasive treatment of idiopathic syringomyelia using myringotomy t-tubes: a case report and technical note.
We present a case of a 44-year-old male without a history of spinal cord trauma, infection, or other pathologic processes, who presented for thoracic pain. Due to progressive pain and left leg paresis, magnetic resonance imaging (MRI) was performed and revealed extensive septated syringomyelia from T5 to T7 and hydromyelia cranially. We applied minimally invasive technique for shunting the idiopathic syrinx into the subarachnoid space using two Richards modified myringotomy T-tubes. Postoperative MRI revealed significant decrease in the syrinx size and clinical six-month follow-up showed improvement of clinical symptoms. T...

“Pseudodoxia Pediatrica”
Under the heading “pseudodoxia pediatrica” Bakwin calls attention to a number of doubtful practices in current use among pediatricians. The worst offender among these, he believes, is indiscriminate tonsillectomy. The American Child Health Association, some ten years ago, surveyed a group of 1,000 children 11 yea rs of age from the public schools of New York City; 61 per cent had had their tonsils removed. The remaining 39 per cent were subjected to examination by a group of physicians who, in turn, selected 45 per cent of these for tonsillectomy and rejected the rest. The rejected children were reexamined b y another ...

Investigating the learning curve in endoscopic compared to microscopic myringotomy and ventilation tube insertion.
CONCLUSION: The absence of a significant difference shows that the learning rates of each technique are statistically indistinguishable. This suggests that surgeons are not justified when citing 'steep learning curve' in arguments against the use of endoscopes in middle-ear surgery. Acute mastoiditis in infants younger than 6  months: is an alternative treatment protocol needed?
ConclusionsAM in children 6  months or younger has similar presentation and characteristics as in older children. Pre-auricular swelling and bilateral AOM are more typical in the younger age band;Streptococcus pneumoniae is the most common pathogen in the younger children. We suggest that the treatment approach should be the same for both groups. (Source: European Archives of Oto-Rhino-Laryngology)

Tympanostomy Tube Innovation: Advances in Device Material, Design, and Office-Based Technology.
CONCLUSIONS: While clinical testing for a number of new technologies is preliminary and ongoing, tympanostomy tube-related innovations hold exciting promise to supplement or potentially replace the present-day armamentarium of tympanostomy tube design and insertion moving forward. Acute mastoiditis in children: contemporary opportunities and challenges.
CONCLUSION: A detailed contemporary description of paediatric mastoiditis presentation and management is presented. The findings broadly mirror those published by other UK centres, but suggest a higher rate of identified disease complications and surgical interventions. Efficacy and Safety in Combining Primary Palatoplasty and Myringotomy in Patients with Cleft Palate
Conclusions: This study documents decreased postoperative care unit time and anesthesia drug exposure without increased length of stay or perioperative complications in patients undergoing combined PP and BMT. These results support combining PP and BMT to reduce exposure to potentially neurotoxic medications and to increase efficient utilization of hospital resources. How Consistent Is Competent? Examining Variance in Psychomotor Skills Assessment
Purpose Direct assessment of trainee performance across time is a core tenet of competency-based medical education. Unlike variability of psychomotor skills across levels of expertise, performance variability exhibited by a particular trainee across time remains unexplored. The goal of this study was to document the consistency of individual surgeons’ technical skill performance. Method A secondary analysis of assessment data (collected in 2010–2012, originally published in 2015) generated by a prospective cohort of participants at Montreal Children’s Hospital with differing levels of expertise was conducted in ...
