The Goat Model for Exclusive Two Handed Endoscopic Middle Ear Surgery Training: A Novel Technique
We describe a novel, exclusively two handed endoscopic approach in an ex vivo animal model for middle ear surgery. The proposed surgical program will guide the trainee endoscopic ear surgeon for a step by step through the main otologic procedures to enhance his or her surgical skills. Is there any association between Helicobacter pylori and otitis media with effusion?
ConclusionH. pylori is one of the important bacteria that plays a role in the pathogenesis of otitis media with effusion. Whether adenoid tissue may be a reservoir for H. Pylori is unclear.

Incidence of Fractures From Perioperative Blood Pressure Cuff Use, Tourniquet Use, and Patient Positioning in Osteogenesis Imperfecta
Conclusions: In pediatric patients with OI, intraoperative use of NIBP cuffs and tourniquets was not associated with iatrogenic fracture. There were no complications related to intra-arterial catheter use. Care should be used during the perioperative period to prevent fractures during body positioning. Level of Evidence: Level IV. Pediatric Emergence Delirium: A Case Study.
Abstract The pathophysiology of emergence delirium (ED) remains a mystery. Using a case study approach, ED is discussed from a clinical perspective. The case is a 4-year-old male who had myringotomy tube placement while anesthetized with sevoflurane. The negative outcome for this child is presented. The epidemiology of the phenomena is reviewed and definitions are examined. Several methods to assess ED are presented, accompanied by a discussion of the development of assessment tools. Research findings are included that address the possible causes of ED including preoperative anxiety, rapid awakening, pain,...

Relation between adenoid size and otitis media with effusion
ConclusionThe relation between adenoids and torus tubarius is more important than the volume of the adenoids. The viscosity of middle ear fluids (serous or mucous) did not influence the rate of treatment efficacy. (Source: European Annals of Otorhinolaryngology, Head and Neck Diseases)

Eustachian Tube Balloon Dilation: Emerging Practice Patterns for a Novel Procedure.
CONCLUSIONS: Eustachian tube balloon dilation is a novel technique for the treatment of chronic ETD, and its role continues to evolve and develop. Current practice patterns demonstrate wide variability in the assessment of ETD, heterogeneity in the timing of ETBD, and controversy in preoperative CT screening. Streamlining grommet pathways for otitis media with effusion and hearing loss in children: our experience.
CONCLUSION: Direct-listing pathways for grommet insertion can reduce waiting times and expedite surgery. Implementation involves a simple alteration of current practice, adhering to National Institute for Health and Care Excellence Clinical Guideline 60. The ultimate decision regarding surgery still rests with ENT specialists. PMID: 30208983 [PubMed - as supplied by publisher] (Source: Journal of Laryngology and Otology)

Tri-dimensional model for ventilation tube permeability
ConclusionIn the printed 3D-model, the volume of most solutions, including water, required to provide enough pressure in order to pass through the VTs was as high as the EAC volume or exceeded it. Soapy water had the highest penetrance while Dexamethasone needed volume of 2.5  ml to pass through the VT, questioning its reliability as a passive drug delivery channel to the ME. Water protection after tympanostomy (Shepard) tubes does not decrease otorrhea incidence – retrospective cohort study
ConclusionWe found that recommending water protection did not have beneficial effect on the incidence of otorrhea after myringotomy with tubes on chronic otitis media with effusion. However, such measures did not appear to have a negative impact on quality of life. This is a populational observational study with few cases (143 cases); these final statements would be better stated by a very large populational study with another large control group.ResumoIntroduçãoA miringotomia para inserção de tubo de ventilação é a cirurgia otológica mais comum. Otorreia é uma complicação frequente deste procedimento e, para ev...

Therapeutic approach to pediatric acute mastoiditis – an update
ConclusionsThe main points of the management protocol are: initiate a broad-spectrum intravenous antibiotic treatment; mastoidectomy should be performed if the infection fails to be controlled after 48 h of administering intravenous antibiotic therapy. We believe that early mastoidectomy prevents serious complications, and our initial observation is that by performing broad mastoidectomy with posterior attic and facial recess exposure, recurrence of acute mastoiditis can be prevented.ResumoIntroduçãoA mastoidite aguda continua sendo a complicação mais comum da otite média aguda. Pode ocorrer também, embora raram...

Do children undergoing bilateral myringotomy with placement of ventilating tubes benefit from pre-operative analgesia? A double-blinded, randomised, placebo-controlled trial.
CONCLUSION: Routine pre-operative analgesia does not reduce pain scores in the early post-operative period. Simple analgesics are effective for rescue analgesia in the minority of cases. PMID: 29998815 [PubMed - as supplied by publisher] (Source: Journal of Laryngology and Otology)
