In-Office Myringotomy and Tympanostomy Tube Insertion
We describe the nuances of performing in-office microscopic and endoscopic myringotomy and tympanostomy tube placement for the treatment of otitis media and Eustachian tube dysfunction. With the advent of new high definition video technology, the endoscope has been safely applied to the clinic setting and is non-inferior to the standard microscopic approach. In this article, we will outline the indications, patient selection, approach selection, topical anesthetic options, procedural details, complications and challenges of in-office myringotomy and tympanostomy tube placement. New perspectives in office-based otoendoscopy and endoscopic ear surgery
Otoendoscopy was first described by Georg von B ékésy and Bruce Mer in the 1950s and 1960s (1,2). Early applications of rigid endoscopic systems included assessment of stapes mobility and direct visualization of the retrotympanic recess (3). In 1982, Yasuya Nomura subsequently developed an angled “needle otoscope” to diagnose middle ear dis ease when placed through a myringotomy in the office (4). These earlier efforts were highly innovative but hampered by limitations in video camera technology. In the 1990s, Muaaz Tarabichi, Jean-Marc Thomassin, and Dennis Poe expanded the use of endoscopes to assist in middle ear ...

When and how to do a myringotomy - a practical guide
Vet Dermatol. 2021 May 6. doi: 10.1111/vde.12966. Online ahead of print.ABSTRACTA myringotomy is a surgical incision made in the tympanic membrane (TM). This gives access to the middle ear for sampling, flushing and instilling topical therapy. It should be considered whenever the TM is intact and there is clinical evidence of otitis media, abnormal TMs and/or abnormal diagnostic imaging. Samples should be collected for cytological investigation and culture, and then the external ear should be cleaned and dried (if required). Myringotomies should be performed under general anaesthesia and, wherever possible, using a video o...

Diagnosis and management of eosinophilic otitis media: a systematic review
CONCLUSION: Intratympanic steroids show the most efficacy in treating EOM, and aggressive optimization of asthma may be beneficial in resolving otologic symptoms. Otologic Manifestations of Eosinophilic Granulomatosis With Polyangiitis: A Systematic Review
Conclusion: Hearing loss and a middle ear effusion are the most common presenting otologic manifestations of eosinophilic granulomatosis with polyangiitis. Sinonasal disease was more prevalent in patients with otologic manifestations. Primary treatment consists of systemic steroids and immunomodulation. M&T, endoscopic sinus surgery, and local steroid administration can be adjunctive measures to alleviate local disease. Level of Evidence: Level 1 (Source: Otology and Neurotology)

Factors Related to Ventilation Tube Extrusion Time in Children and Adults
CONCLUSION: History of VT insertion and VT size were significantly related to VT extrusion time in both children and adults and defined as factors associated with extrusion before 12 months in children. The findings suggest avoiding VT with a diameter < 1 mm and considering an appropriately larger size in patients with a history of VT insertion to optimize VT retention.PMID:33720800 | DOI:10.1177/0145561321999204 (Source: Ear, Nose and Throat Journal)

Pediatric Otogenic Cerebral Sinus Vein Thrombosis and Thrombophilia.
CONCLUSIONS: Our findings demonstrate good outcomes for otogenic CSVT treatment with intravenous antibiotics, anticoagulation, and conservative surgical intervention, which supports the current trend in management. The prothrombotic evaluation revealed transient inflammation-related risk factors but did not alter management. Further prospective multicenter studies are needed to determine its relevance. Myringotomy and insertion of grommets as day surgery: a case study.
Abstract This evidence-based case study follows a child from a nursing assessment on the day of his elective surgery at a children's hospital for myringotomy and insertion of grommets under general anaesthesia through to his arrival at the operating room. Potential pre-operative problems are identified and two problems that arose are discussed in detail. The main care provider in this case was a student nurse referred to as 'the nurse', supported by a qualified nurse, referred to as the 'registered nurse', who performed some assessments.
