Learning Curve of Two Handed Endoscopic Ear Surgery on Sheep Temporal Bone: A Fellow ’s Perspective
AbstractThe study puts forth the learning curve of two handed endoscopic middle ear dissection on a sheep temporal bone. It also highlights the usage of sheep temporal bone as an effective simulation technique for two-handed cadaveric temporal bone dissection. 6 sheep temporal bone models were dissected in a period of 15  days. Harvesting of cartilage graft, endoscopic myringotomy and grommet insertion, cartilage butterfly tympanoplasty, ossiculoplasty, stapes surgery and facial nerve decompression, using the Endohold®, were conducted on each bone. There was a remarkable decrease in the time required for subsequen t temp...

Helping Children with Special Needs: Who Receives Tympanostomy Tubes?
CONCLUSION: The population of children with developmental disorders undergoing myringotomy or tympanostomy tube placement has a different demographic composition than the general population and faces distinct financial and insurance coverage burdens. Further study should be done to assess if these differences impact long term outcomes. PMID: 33455429 [PubMed - as supplied by publisher] (Source: The Annals of Otology, Rhinology, and Laryngology)

Adenoid Morphology and Other Prognostic Factors for Otitis Media with Effusion in School Children
ConclusionSpecific morphology of adenoid enlargement and tubal tonsil enlargement was more critical in predicting the risk of developing OME and not just the grade of adenoid hypertrophy. The microbial flora in the adenoid core was similar in children with and without OME. (Source: Indian Journal of Otolaryngology and Head and Neck Surgery)

Utility of gastric ultrasound in evaluating nil per os status in a child
We present a 4-year-old patient who presented for bilateral myringotomy with tympanostomy tube insertion, who was found to have evidence of a full stomach during preoperative gastric ultrasound examination. The use of preoperative gastric point-of-care ultrasound in evaluating stomach contents and confirming NPO times is reviewed and its application to perioperative practice discussed. Triple Canal Occlusion for the Treatment of Intractable Menière's Disease
Conclusions: Based on our limited experience and the early reports in the literature we consider that there are potential patient benefits for triple canal occlusion for intractable Menière's disease as an alternative to vestibular neurectomy due to the reduced morbidity and long-term efficacy. Facial nerve baroparesis during airflight: a case report and literature review.
CONCLUSION: Facial nerve baroparesis is a mostly temporary rare phenomenon that can be managed effectively with ventilation tube insertion. In the event of long-standing facial nerve palsy after descent of the aircraft, urgent myringotomy should be performed to prevent permanent facial nerve damage. Adult Cleft Palate Repair: It's Worth a Shot.
We present a case of a 66-year-old woman who underwent secondary cleft palate repair. The patient was born with a cleft palate and at age 15 years underwent palate repair that subsequently broke down. She had profound velopharyngeal incompetence, was difficult to understand in conversation, and had a long history of hearing issues requiring hearing aids. She underwent revision palatoplasty and insertion of bilateral grommets. Postoperatively she had marked improvement in her speech, hearing, and quality of life. This case demonstrates the utility of secondary repair of cleft palate in the adult population. PMID: 33...

Factors associated with the development of paediatric chronic otitis media by age nine: a prospective longitudinal cohort study of 6560 children.
CONCLUSION: The association between chronic otitis media and otorrhoea, abnormal tympanograms and grommets supports the role of the Eustachian tube and otitis media (with effusion or acute) in the pathogenesis of chronic otitis media. The role of snoring, adenoidectomy and tonsillectomy is unclear. Associations suggested by previous studies (sex, socioeconomic group, parental smoking, maternal education, childcare, crowding and siblings) were not found to be significant predictors in this analysis. Successful treatment of highly recurrent facial baroparesis in a frequent high-altitude traveler: a case report
ConclusionsPrompt PET insertion may represent the preferred treatment for individuals who suffer recurrent episodes of facial baroparesis. Education regarding this rare condition may prevent unnecessary testing and treatment of affected patients. Future studies should explore the pathophysiology and risk factors, compare therapeutic options, and provide follow-up data to optimize the management of affected patients. Reducing antibiotic prescriptions in children is not associated with higher rate of complications
Conclusion: ProBA project implementation —recommending 5 days of amoxicillin for AOM when needed and 6 days of amoxicillin when streptococcal pharyngitis is detected—was associated with a reduced antibiotic use without an increase of complications. What is Known:•Acute otitis media (AOM) and streptococcal pharyngitis are common pediatric infections and frequent cause of antibiotics prescription.•Fear of rare complications like mastoiditis and acute rheumatic fever can hinder health professionals ’ compliance with evidence-based guideline. What is New:•Guidelines recommending a short course of antibiotics for AOM ...

Considerations in Management of Acute Otitis Media in the COVID-19 Era.
CONCLUSION: The case of a patient with active COVID-19 presenting with complicated acute otitis media in whom middle ear fluid was sampled to evaluate the etiology of the infection and the potential middle ear predilection of SARS-CoV-2 is described. This study has implications for the clinical management of patients with both known and unknown SARS-CoV-2 infection who present with ear disease. While middle ear suctioning may not be aerosol-generating, the risk of coughing or prolonged close contact requires heightened precautions during otologic procedures in patients with suspected or confirmed COVID-19. Provision and Utilization of Team- and Community-Based Operative Care for Patients With Cleft Lip/Palate in North Carolina.
CONCLUSIONS: American Cleft Palate Craniofacial Association -approved team-based care accounts for the majority of cleft-related care in North Carolina; however, a substantial volume of cleft-related procedures was provided by community providers, with 3 providers accounting for the vast majority of community cases.
