Systematic Review and Meta-analysis of the Efficacy of Oral Medications Compared with Placebo Treatment in the Management of Postherpetic Neuralgia.
CONCLUSION: This review has provided favorable but low-quality evidence to support the use of anticonvulsants for PHN. Although statistically significant effects were observed for posttreatment pain and the percent of responders, the number of studies in each subgroup analysis for anticonvulsants was small and the included studies had high or unclear risk of bias. Further high-quality methodologic studies are needed to explore the effects of orally administered anticonvulsants for PHN. CONCLUSIONS: Anticonvulsants appear to be both effective in reducing migraine frequency and reasonably well tolerated. There is noticeable variation among individual agents, but there are insufficient data to know whether this is due to chance or variation in true efficacy. Acetazolamide, clonazepam, lamotrigine and vigabatrin were not superior to placebo (one trial each). Relatively few robust trials are available for agents other than sodium valproate/divalproex sodium and topiramate; gabapentin in particular needs further evaluation. CONCLUSIONS: NORSE as the presenting symptom of CAA has been reported once but not in absence of CMB.There were other prominent psychotic features which als occurred prior to appearance of CMBs. Cortical hemorrhages appeared after, and probably as a consequence of PRES.The presentation and subsequent clinical course of this patient raised the concern for rapidly progressive dementia and a brain biopsy earlier in the clinical course might have helped in early diagnosis. The rationale, design, and progress of two novel maintenance treatment studies in pediatric bipolarity.
DISCUSSION: Since the blind has not been broken on either of these clinical trials, conclusions about the maintenance effectiveness of either VPA or Li+ in youths with bipolar disorder type 1 or 2 cannot be made yet. Similarly, it is unclear whether VPA is superior to placebo in genetically high-risk youths with cyclotaxia.
