candida fungal infection with stimulator
Had a weird case a couple of weeks ago. My partner, who is an outstanding technical surgeon for implants (one of the best I have seen) had a stim follow up while he was on vacation. The wounds were dehiscing and the incisions looked odd (my partner has had only two infections over a 25 year career, so he is pretty damn good). I took her to the OR and removed the stim, packing the incisions open. Initial cultures were negative, but eventually grew candida. Upcoming DAT - Type 1 Diabetic
I hope everyone is doing great! As I prep for the DAT, I've been looking into the 'processes' involved for me taking the exam as a type 1 diabetic and what I need to do to make sure all is okay with items such as insulin pump/having a juice for potential low blood sugar, etc. I know they have to be pretty tight on rules (no phones etc.) but I just want to make sure all would be okay if for instance my insulin pump was alerting me on an upcoming low blood sugar and I had to click the button... MS1-MS2 doesn't teach you bread and butter medicine in enough detail
Just reflecting that in ms1-ms2, I really think that there was excessive emphasis on zebras without enough relative emphasis on day to day medicine. Learning about conditions that are 1 in 1 million really doesn't carry the same value as learning about diabetes. And quite frankly a small chunk of First Aid teaches you virtually useless facts that have literally 0 clinical utility, ever. The solution is to get rid of that chunk, maybe 10-15% of it. nodal relapse 9y after RP and PORT for prostatic carcinoma
75 years old patient, with controlled diabetes and hypertension, very fit. he underwent in 2009, a radical prostatectomy followed 30 fractions of adjuvant 3D conformal radiation therapy to the prostatic bed only. in 2018: PSA rise to 2.16 2019: PSA 2.16 Pet PSMA showed a 7 mm LN at the level of L4-5 secondary lesion, with no recurrence in the tumor bed, neither in bone or other LNs. When to refer to specialist?
I am a resident in midwest unopposed program, small town. We have specialists in some areas (cardiology) but not others (endo) and it has shaped our referral patterns and training for common stuff pretty heavily. For example, folks in my program don't touch cardiac meds in chf patients because "cardio is already on board." There isn't an endo for hours out, so we tend to take on some of the more challenging diabetes cases because of lack of access. I'm not 100% convinced we do a great job... How do you deal with those pain sesking patients?
I have this 20 year old male on dialysis secondary to uncontrolled diabetes who came in with intractable abdominal pain nausea and vomiting.Dont get me wrong hes not on any chronic opiod regimen so dont want to jump to conclusions but his pain is 20/10 getting fentanyl 25 q3 hrs. Ct abdomen completely normal. Egd normal. BP normal, Labs nml Tried reglan for concern for gastroparesis yet nothing.
