How much medical management (HTN, diabetes, HLD, etc) do you do on inpatient psych? Did you do any of it as a resident (after off-service IM)?

POC Glucose for diabetics in outpatient setting
Currently practice in ASC where, by policy, all diabetics get a FS glucose check and generally canceled if FS >250. Moving to outpatient hospital procedure suite. How many of you are routinely checking FS glucose in diabetics? Are you only checking in the setting of steroid injection? What's your cutoff number to cancel the case? Literature indicates that rise in BG is about 100 and lasts 2-3 days. (Source: Student Doctor Network)

Poly pharmacy and falls
Long story short, I had an older patient on sedating meds which I had been working to minimize. Last time I saw her she was having poor PO intake and I focused on enhancing that and getting her to be medically evaluated in the ER because she is also diabetic on insulin (which she did not follow through on, and it was telemedicine so I could not arrange direct transport). In hindsight I should have probably lowered her meds because blood levels could have been going up. She then proceeded to... Read more (Source: Student Doctor Network)

How to categorize virtual patient education?
Hi everyone, I am currently volunteering with a free clinic to provide virtual diabetes/nutrition education to Spanish Speaking patients. I give the presentation to the clinic's patients and also serve as an interpreter for the clinic's nutritionist. During these meetings, I get to really learn about the patients as they ask us different questions pertaining to their conditions (5 people max show up so essentially just hearing out patients and trying to help them with their nutrition and... Blood glucose level to cancel cataract surgery
Thoughts on this? What would be your BG range and cut-offs? Recently I cancelled one whose BG was 400-500 mg/dL. Pt claimed to be compliant with all her diabetic medications. Talked to her PCP and found out she was lying and that she actually ran out of her humalog a month ago. No recent A1c.. given the hx I can assume her BG in the 400-500's range for the past month. Not the healthiest patient either, with CHF exacerbation in the last 3 months, known CAD, morbidly obese. Known history of... Read more (Source: Student Doctor Network)

AMA releases document on appropriate language and health equity
https://www.ama-assn.org/system/files/ama-aamc-equity-guide.pdf The AMA recently released this equity guide focused on how words nobody finds problematic are now problematic. For example, the term "diabetic" is dehumanizing - the correct term is "person with diabetes." I wonder if this was concluded after surveying diabetics and finding that the majority consider the term dehumanizing, or because a small handful of academics decided it was. The term "whitelist" or "blacklist" is also... Read more (Source: Student Doctor Network)

Modifier 25 use question- example
I'm having a debate with another pod buddy of mine regarding use of a 25 modifier in this situation- A patient returns to your office your treatment of a his diabetic foot ulcer. You perform a debridement and after you measure you realize that the wound hasn't progressed. You evaluate the patient's shoe gear and the rest of the limb for any ASOI or ischemic changes. You call the patient's home nursing service and give new orders verbally. My belief is that all you can bill for is the... Read more (Source: Student Doctor Network)

https://www.cms.gov/outreach-and-education/medicare-learning-network-mln/mlnproducts/downloads/globallsurgery-icn907166.pdf You're on call. A new patient shows up your list - admitted septic overnight. Some sort of forefoot gangrene, meth injecting, diabetic, vasculopath, smoker train wreck. Or perhaps another DPM already took a crack at a partial amp or toe amp and the whole thing fell apart (and they don't have priviledges at your hospital or the patient was transfered to you...
