Euvolemic hypernatremia
Hi everyone, I'm having a hard time understanding this notion: In patients with diabetes inspidus, there is a loss in free water, which results in hypernatremia (that, I can understand). However, why is the urinary sodium excretion also high? In my understanding, if there is a loss in free water, the body would compensate by increasing aldosterone secretion and reducing ANP secretion => less sodium excretion => more sodium reabsorption, water follows => euvolemic hypernatremia. Using Zyprexa in someone you suspect has Diabetes?
Hi, I've been trying to see some intakes at a local center where we don't have ability to get labwork done before patients are seen with us. Typically, we start meds then tell patient's to get their bloodwork done by their future providers. I was wondering, considering how wonderful Zyprexa has worked, is it possible to have the patient start the medication (depending on severity) and have the ongoing provider or PCP just check labs? ASA status
Not the most important questions but I am curious as to what the community would give to these patients in terms of ASA score. ESRD, Diabetes type 1 insulin dependent I'm curious about above because some people give ESRD an ASA 4 simply due to ESRD, because dialysis is life saving. But the diabetic type 1 patient gets an ASA 2 even though insulin is life saving. Both diseases cause significant consequences later on in life. What are your thoughts?
