Roles of Integrin in Cardiovascular Diseases: From Basic Research to Clinical Implications
Int J Mol Sci. 2024 Apr 7;25(7):4096. doi: 10.3390/ijms25074096.ABSTRACTCardiovascular diseases (CVDs) pose a significant global health threat due to their complex pathogenesis and high incidence, imposing a substantial burden on global healthcare systems. Integrins, a group of heterodimers consisting of α and β subunits that are located on the cell membrane, have emerged as key players in mediating the occurrence and progression of CVDs by regulating the physiological activities of endothelial cells, vascular smooth muscle cells, platelets, fibroblasts, cardiomyocytes, and various immune cells. The crucial role of integ...

Association of device detected atrial and ventricular tachyarrhythmia with adverse events in patients with an implantable cardioverter ‐defibrillator
ConclusionOur findings from a large cohort of ICD recipients enrolled in landmark clinical trials show that device detected AF and VT/VF can be used to identify patients with increased risk for CHF exacerbation and mortality. These findings suggest a need for early intervention in CHF patients who develop device-detected atrial and ventricular tachyarrhythmias. (Source: Journal of Cardiovascular Electrophysiology)

Atrial Fibrillation Ablation in Patients with Pulmonary Hypertension: Multicenter Experience
Pulmonary hypertension (PH) is a progressive condition characterized by pulmonary vascular remodeling and elevated pulmonary arterial pressure. Atrial arrhythmias are common in PH and may contribute to right ventricular failure and death1,2. We retrospectively identified patients with pre-capillary PH who underwent catheter ablation for AF at four high-volume ablation centers. Included patients had an established clinical diagnosis of PH, with suspected pre-capillary etiology, and at least one right heart catheterization showing mean pulmonary artery pressure>20 mm Hg and pulmonary capillary wedge pressure ≤15 mm Hg. 2024 European Heart Rhythm Association/Heart Rhythm Society/Asia Pacific Heart Rhythm Society/Latin American Heart Rhythm Society expert consensus statement on catheter and surgical ablation of atrial fibrillation
In the last three decades, ablation of atrial fibrillation (AF) has become an evidence-based safe and efficacious treatment for managing the most common cardiac arrhythmia. In 2007, the first joint expert consensus document was issued, guiding healthcare professionals involved in catheter or surgical AF ablation. Mounting research evidence and technological advances have resulted in a rapidly changing landscape in the field of catheter and surgical AF ablation, thus stressing the need for regularly updated versions of this partnership which were issued in 2012 and 2017. Seven years after the last consensus, an updated docu...

Troponin Elevation in Patients Undergoing Percutaneous Hepatic Perfusion for Metastatic Uveal Melanoma
CONCLUSION: Percutaneous Hepatic Perfusion is safe and associated with a transient, asymptomatic troponin elevation peri-operatively without major adverse cardiac events at 90 days. The observed troponin elevation is likely secondary to coronary demand-supply mismatch related to procedural hemodynamic shifts, hypotension, and anemia.PMID:38605434 | DOI:10.1177/10732748241246898 (Source: Cancer Control)

Can transesophageal echocardiography be safely omitted in patients scheduled for elective ablation of atrial arrhythmias? Data based on the LATTEE registry
CONCLUSIONS: Simple echocardiographic parameters could help identify individuals for whom TEE could be safely omitted before scheduled for elective CA due to atrial arrhythmias.PMID:38606739 | DOI:10.33963/v.phj.100081 (Source: Kardiologia Polska)

Non-valvular atrial fibrillation in patients on peritoneal dialysis, prevalence, treatment and professionals involved
The objective of our study was to know the prevalence, treatment and professionals involved in the management of atrial fibrillation in peritoneal dialysis patients. For this purpose, we performed a descriptive analysis through a survey sent to different peritoneal dialysis units in Spain. A total of 1,403 patients on peritoneal dialysis were included in the study, of whom 186 (13.2%) had non-valvular atrial fibrillation. In addition, the assessment of the scores of thromboembolic and bleeding risks for the indication of oral anticoagulation was mainly carried out by the cardiologist (60% of the units), as well as its pres...

Remimazolam to prevent hemodynamic instability during catheter ablation under general anesthesia: a randomized controlled trial
CONCLUSIONS: Remimazolam was a viable option for general anesthesia for cardiac ablation. Remimazolam-based total intravenous anesthesia was associated with significantly fewer hypotensive events and vasopressor requirements than desflurane-based inhalational anesthesia was, without significantly more complications.STUDY REGISTRATION: ClinicalTrials.gov (NCT05486377); first submitted 1 August 2022.PMID:38609684 | DOI:10.1007/s12630-024-02735-z (Source: Canadian Journal of Anaesthesia)

Empowering Little Fighters: Post-Cardiotomy Pediatric ECMO and the Journey to Recovery
CONCLUSION: The early initiation of ECMO for paediatric cardiotomies could be a beacon of hope for families and medical teams confronting these challenging situations. Improvement in indicators of adequate perfusion and ventricular recoveries like pH and serum lactate and absence of arrhythmia and sepsis are associated with good outcomes.PMID:38607876 | DOI:10.4103/aca.aca_184_23 (Source: Annals of Cardiac Anaesthesia)

The Impact of Valve Surgery on the Safety and Cardiac Function of Patients with Valvular Heart Disease Complicated by Pulmonary Arterial Hypertension
CONCLUSION: Valve surgery in valvular heart disease with PAH is influenced by patient age, functional status, and PAH severity. Despite advances in surgical techniques, there remains a notable gap in understanding the nuanced interplay between these conditions and the variable outcomes of valve surgery. This study addresses this research gap, offering comprehensive insights into how age, heart function, and PAH severity influence postoperative outcomes. These findings are crucial for clinicians, providing a more informed basis for tailored treatment strategies, and ultimately enhancing patient care in this complex clinical...
