Radiation therapy for intracardiac metastasis
Dear colleagues , my patient 56 yo known to have a history of oligometastatic STS, recently presented with a secondary lesion in the heart. He was strictly asymptomatic. we decided to go forward with systemic treatment not considering radiation in this setting. Unfortunately, he passed away secondary to arrhythmia (most probably related to his lesion). 1- i would like to know about your experience in the indication of radiation therapy (SBRT or other ) to treat similar lesions? selection... Is it possible in non-invasive cardiology private practice to see a majority of cases of arrhythmias and to a lesser extent ischemic cardiomyopathies?
Hello everyone, I am a medical student and I am wondering about my future specialty. I really like non-invasive cardiology and I would like to work in the future in private practice. The subject I am passionate about is arrhythmias and less so myocardial infarctions. My question is: is it possible in private practice to see a majority of cases of arrhythmias and to a lesser extent ischemic cardiomyopathies? Thanks in advance Mark (Source: Student Doctor Network)

Antidepressants in patient with arrhythmias
I haven't found much data to suggest which medications might have the best safety profile, with the exception of recommendations to avoid TCAs and possibly celexa. I also see mirtazapine is associated with potential for slight increase in qtc, but nothing concrete in terms of guidelines. How would you handle a patient with history of arrhythmia on rate control? Research Fellowship - Please help!
Hello everyone, I am an IMG with low scores (Step 1: 212, CK: 222, CS: pass, Step 3: 199) all on first attempt. I have 5 USCE (University of Miami, Northwestern and Cleveland Clinic FL) and received 5 LORs. Had 5 interviews for Internal Medicine, but did not match this cycle. Now, I have been accepted in 2 different research fellowships and I have to decide between those two. One is in Mass General (Harvard) with the founder of the arrhythmia service in Harvard (I want to be a Cardiologist),... Research Fellowship - Please help! (Source: Student Doctor Network)

Any of you find ST Map, or other features on Philips monitors useful
I was looking over the Philips IntelliVue monitor user manual. There are a lot of features that I haven't seen used much. Sure, the calculator, and drug calculator are probably obsolete when most places have EMR or smart phones. Have any of you guys found it useful to use ST Map, QT analysis, Arrhythmia analysis, etc. on these monitors? I'm working on a presentation for my residency and want to review ECG, SpO2, waveforms, etc., and thought it may be at least somewhat interesting to go... Few Questions for a Thoracic Surgeon
Hey there, I'm an M3 considering an integrated I6 program and had a few questions, have read the FAQ and most of the thoracic surgery threads here but haven't seen an answer to these - How much arrhythmia surgery does the average community CT surgeon do? Do they routinely implant pacemakers and follow up with the device interrogations? - Would an I6 program adequately train you in advanced endobronchial oncology techniques like laser therapy, cryotherapy or stent placement? - Do most... Syncope dogma
Is anyone aware of any actual data that the usual dogma of "presence of prodrome" is more likely to exclude cardiogenic/arrhythmogenic syncope? This seems kinda made up - I have had a bunch of patients with a malignant arrhythmia without syncope and they certainly have exactly the same prodrome and symptoms that are supposed to make me feel better about that spell not coming from their heart. What is your practice with syncope where you work? Do you admit these for obs? Discharge with f/u?... Syncope dogma (Source: Student Doctor Network)

Considering leaving medicine, please help?
Hi Everyone, I'm in a bit of a dilemma. I hope you can give me some advice. During my 3rd year of medical school, I realized that clinical medicine/mortality/morbidity caused me to develop incredible anxiety, panic disorder, and an arrhythmia 2/2 to the anxiety. Being on wards makes me hyperventilate and brings on this feeling of impending doom. I have worked incredibly hard (board scores in the 250s) and it is heartbreaking that I cannot seem to cope with medicine. I have rotated in and... Considering leaving medicine, please help? A question in arrhythmia
Hi, i'm 3rd year and I have a question about arrhythmia, we studied that arrhythmia could be as a result of disturbance in Automaticity / Excitability / Conductibility. As I know, Disturbance in auto. could be Sinus bradycardia and sinus tachycardia. Disturbance in conduct. = AV bock( types) but about excitability, I understood that there is Extra systole and Reentry i got that examples of extra systole is Atrial extra systole or vent. or atriovent. and example of reentry is Atrial flutter... Clinical question of the week: How would you treat this patient w palpitations, dyspnoea, dark stools?
A 75-year-old male with a prior history of hypertension, diabetes and treatment for “arrhythmia” presented to the emergency department complaining of palpitations and dyspnea on exertion over the last two days. The patient also reported dark stools over the last two days. No other symptoms were reported. Current medications included atenolol 50 mg qD, warfarin 5 mg qD, metformin 850 mg BiD.   The patient did not remember his last INR result, but reported no blood tests over the last two months. On examination the heart rate was 140, irregular, BP was 90/70, respiratory rate 22, SatO2: 94%. The patient ...
