USAP Colorado
The rumor mill has it that the Denver USAPs and maybe others in CO are about to implode due to the FTC lawsuit against USAP. Let’s watch out for locums assignments there. Although Denver is not known for good pay there may be some upcoming good opportunities due to a quick exit of USAP!! (Source: Student Doctor Network)

RFA needle sizing for local anesthesia (without sedation)?
At my current setup, we only offer local anesthesia (no sedation) for procedures. RFA cannula sizing is 20g (lumbar) and 22g (cervical), MDT Accurian (no cooled RF setup). Needles are placed parallel to medial branch nerve with 90 seconds initial burn followed by a 180 degrees rotation for a 2nd burn. Patients do well with local and don't need sedation. Results are pretty good but I'm always wondering if I could optimize the outcome more by upsizing the needle gauge. For those... Read more (Source: Student Doctor Network)

Narcotic administration in cardiac surgery cases
I am curious what everyone is doing currently for narcotic administration in cardiac surgery cases? Are people using infusions or just giving boluses before times of stimulation? Unfortunately sufantanil has been available for a long period of time. When I was a medical student and starting residency some attendings were still giving 2mg/kg of morphine. That later evolved to giving 5000-6000 ug fentanyl and later 1000-1500 mg sufentanil. When ERAS and early extubation became popular... Read more (Source: Student Doctor Network)

USAP-answer to cheaper labor
From Richard Dutton himself on dealing with staffing issues. At least admitting they’re looking for cheap labor. Again, these models crack with no surprises act and increasing labor cost (for both private equity and the pyramid MDs) How is USAP addressing the continually growing anesthesiologist shortages in the U.S.? RD: We are continuously investing in recruiting and retention efforts. I'm most proud of our work establishing anesthesia residencies in four of our cities and are... Take Medicine Back
If any of you guys are interested there is a small grassroots movement that’s been around for a couple of years made of physicians and some lawyers helping us who’s mission is to try and take Medicine back from PE and other corporate entitities. There is a FaceBook Group as well as a Website. It discusses the Corporation practice of Medicine and how it’s illegal in many states but of course ignored in the interest of money. I did a virtual conference w them two weekends ago and it was... Read more (Source: Student Doctor Network)

How long to stay at first job
I am a new attending who recently started working out of fellowship. Due to personal goals, I am now looking to move locations, but I am concerned about seeming flaky if I leave this position too soon. Is there a safe amount of time before it wouldn't seem unreasonable to start looking for new employment? (Source: Student Doctor Network)

Any private equity lobby on reimbursement?
Just wondering. Since PE really likes anesthesiology practices, and we have obvious problems in anesthesiology, such as low medicaid/medicare reimbursement rates compared to other specialties, have they or big anesthesia companies ever try to lobby on increased reimbursements for medicaid/medicare? We like to say its the ASA's job on these forums, but the truth is ASA is tiny. Their funding is also tiny. Feels to me PE can benefit a lot if anesthesiology medicaid/medicare reimbursements are... Billing E/M or therapy codes with ECT?
Hey everyone, I'm a PGY-4 on an ECT rotation and had a question about billing. Bit of a preamble first: Our service starts at 7am with pre-evaluations including interval H&P, medication reconciliation, assessment of side effects from ECT, and evaluation for adjustment of dose or electrode placement. 6-10 patients per morning until about 11am-12pm depending on anesthesia. We then perform the procedures and end up having to write both (1) an Interval HPI note (indicating history assessments... Read more (Source: Student Doctor Network)

News from AAOS
Orthopedic Surgeons on Track to Earn Less Than Taco Bell Workers If current trends continue, patients are in big trouble www.medpagetoday.com "If those numbers aren't disappointing enough for young surgeons who've already spent a dozen years in training, the same group fit a third-order polynomial model to historical data for Medicare reimbursements for arthroplasties, yielding a projection out to 2030. Actual figures from 2007 to 2023 showed mean reimbursements falling over time, and falling faster with each passing year from 2019 onward. T...

GI fellowship elective/away rotation in Japan?
I'm a current GI fellow and want to spend some time in Japan. I think that it'd be very interesting to see how medicine is practiced there and see how they do things differently in terms of management. Already, I can tell that people who train in Asia are much more talented at unsedated colonoscopies than in the US where we can rely on anesthesia. I have elective time, and I'm sure that I can convince my program to allow me to do an international elective in Japan if such a thing exists. I... Read more (Source: Student Doctor Network)

MS4: IM- > Anesthesia or Buyers Remorse?
Hey all, I'm a MS4 who applied IM. I'm a strong applicant (got IVs at all the 'Big 4') and was deadset on Pulm/CC Unfortunately my school has very limited elective time and I had zero exposure to anesthesia until the last couples months. Always been interested in procedures so took an anesthesia rotation and got to work with some awesome anesthesiologists. Had a blast on cardio thoracic. Anesthesia is essentially everything I love about crit care, the technical aspects are awesome, the... Read more (Source: Student Doctor Network)

Administrators meddling in OR operations
This is mostly for the more seasoned folks with admin experience..... Having ran ORs/Led a few departments of varying sizes the issue of administration (ie CMOs, CEOs, etc.) and some "protected" surgeons (who are plugged into the hospital board etc) meddling in OR operations thinking they have the answers and pushing chairs, floor runners etc etc around is (IMO) is a growing trend across the board.... for those who were able to successfully mitigate this I am curious to hear as to how it... Attention financial enthusiasts: The Secure 2.0 Act
The SECURE 2.0 Act was passed into law in 2022, but takes effect as of 2024. This act allows funds from a 529 account to be transferred TAX-FREE to a Roth IRA for the beneficiary of the 529 account. This essentially allows you to kickstart a beneficiary’s Roth IRA savings in addition to the educational spending benefits that you get from a 529 plan. Important to know for those who aren't familiar with 529 plans is that the money contributed is post-tax, so for many of us this will... Read more (Source: Student Doctor Network)
